About us
Ulverston Medical Practice are a high achieving practice, an integral part of the local community and located in Ulverston on the edge of the Lake District in Cumbria. We are a training practice and actively involved in our local P.C.N. and Federation. Currently we have six Partners, four Salaried GP’s a wonderful nursing team of 5 practice nurses and 4 HCAs, and highly supportive admin and management team. We have a wide range of roles across the PCN who work regularly in the Practice including pharmacists, mental health practitioners, social prescribers and physiotherapists.
Job Summary
To provide and maintain an extremely high standard of nursing care for patients as well as providing nursing assistance to the doctors and other members of the primary healthcare team. The duties will include all tasks normally undertaken and in addition any roles agreed between the nurse and the doctors as appropriate, having regard to current training. Take responsibility for chronic disease registers including QOF and work with admin staff to maintain robust call and recall systems for chronic disease, cervical cytology, immunisation and other population based health promotion and disease prevention care programmes, escalating where necessary patients who need intervention from a GP to ensure a review as appropriate, constructing care pathways where patients receive appropriate investigations and reviews by an appropriate clinician at the appropriate time interval.
Job Description
Professional
- Offer advice about childhood and adult vaccinations and ensure vaccines are administered under patient group directions.
- Running well person clinics and health promotion programmes by providing a holistic assessment and lifestyle advice on diet, smoking, alcohol intake and exercise. Facilitates change.
- Assessing problems presented opportunistically by patients.
- Able to document Consultations according to NMC guidelines
- To lead and support all staff in managing chronic diseases and achieving QOF and other practice targets
- Ensures awareness of statutory and local clinical protection procedures, including systems of referral.
- Ability to recognise signs and symptoms of child abuse.
- Perform a holistic assessment of patients attending for cervical cytology smear tests.
- Perform venepuncture according to local guidelines
- Provide a holistic approach to wound management and implement wound care in line with current evidence-based guidelines.

Person Specification
Qualifications - Essential
- Registered General Nurse
- Full registration with NMC
- Experienced General Practice Nurse, including the knowledge and understanding of the range of primary care and community services
- Specific nursing skills/qualifications relevant to the post, including chronic disease management, cervical cytology, wound care, childhood immunisations, travel clinic etc
- Experience of QOF and chronic disease management (such as Diabetes, CHD, Asthma etc)
- A demonstrable commitment to professional development
- Competency in basic nursing duties required for the post
- Excellent communication and interpersonal skills
- Willingness to contribute to multi-professional working
- Ability to work well autonomously and within a team
