Full job description

The Nelson Medical Practice is a modern practice based in Wimbledon Chase and we have an exciting opportunity for a Patient Care Navigator to join our team. This is a great opportunity to work in an innovative and fast expanding GP Practice in Merton. We are passionate about providing high quality patient centered care to our 33,000 patients. We currently have 1 vacancy covering a variety of shifts. Working a mixture of mornings and afternoons on the following shift pattern :-

Monday 8am - 2pm

Tuesday 12.30pm - 6.30pm

Wednesday 12.30pm - 6.30pm

Thursday 8am - 2pm

Friday 8

Totaling 30 hours per week

Good communication and customer skills are essential as this role involves dealing with members of the public both on the telephone and face to face, good IT skills are also essential. Experience is not vital as training will be given.

Job Summary:

To assist the Dr's and other Clinicians in the delivery of high quality service
Receiving and directing patients on arrival at the Practice in a courteous and efficient manner
Answering telephone calls coming into the Practice and dealing as appropriate
Giving results to patients
registering new patients onto the Clinical system
Booking appointments
attending relevant meetings and staff training
Answering general queries and explaining processes

Person Specification:

Good IT skills
Good communication skills
Good organisational skills
Ability to multi - task
Ability to work to deadlines

Job Types: Part-time, Permanent

Pay: £13.51 per hour
