Job Summary
We are seeking a dedicated and highly skilled Pharmacist to join our healthcare team. The successful candidate will be responsible for dispensing medications accurately, providing expert advice on medication use, and ensuring optimal patient care. This role offers an opportunity to work within a dynamic environment where patient safety and care are paramount. The ideal applicant will have relevant experience, excellent communication skills, and a strong commitment to healthcare excellence.

Responsibilities

Dispense prescribed medications accurately in accordance with legal and organisational guidelines
Provide expert advice to patients regarding medication usage, potential side effects, and interactions
Monitor patient responses to medication therapies and adjust recommendations accordingly
Collaborate with healthcare professionals to develop comprehensive patient care plans
Maintain meticulous records of prescriptions, patient interactions, and inventory management
Ensure compliance with all legal, safety, and professional standards related to pharmacy practice
Educate patients on health and wellness topics as appropriate
Stay updated with the latest developments in pharmacology and healthcare regulations
Ensuring daily claims are completed in a timely manner
MDS trays are handled efficiently and accurately to ensure our patients receive medications correctly and in a timely manner
End of month claiming through portal
Handling Pharmoutcomes efficiently in a timely manner to ensure patient care is met in a timely manner
Monitoring services and DMS referrals
Maintaining relations with other health care professional, carers, nurses, hospitals, care homes and GP's

Skills

Proven experience in community settings is highly desirable, demonstrating ability to interact with patients.
Strong commitment to patient care with excellent interpersonal skills
In-depth knowledge of anatomy and physiology relevant to pharmacology
Proficiency in medication administration procedures and protocols
Exceptional organisational skills with attention to detail for accurate dispensing and record keeping
Ability to work effectively within a multidisciplinary healthcare team
Excellent communication skills, both verbal and written, tailored to diverse patient populations
