Job Summary

Carer Support Navigator will offer carers assessments and support to carers in Northamptonshire to enable the sustainment of the caring role, reduce the impact of caring on carer’s health and wellbeing and to prevent crisis. By ‘carers’ we are referring to informal carers who look after a friend or family member with a disability or long-term illness out of a sense of duty and compassion as opposed to a care worker who is employed in their role. This role will include being a part of our Carers Support Line where you will be supporting carers primarily by telephone (both incoming and outgoing calls) and via email alongside working within our community team to provide the best possible support and outcomes for Carers.

Job Dimensions

1. To provide an informed and professional response to carers through a confidential service of carer-centred assessment, support and crisis planning via our Carers Support Line.

2. Provision of advice and information alongside emotional support.

3. Provide short term intervention plans for Carers where appropriate ensuring they are fully supported by effectively managing risk and responsibilities, using a person centred approach.

4. To maintain accurate written and concise computer records in accordance with local policies and data protection and confidentiality legislation.

5. To manage own caseload and have the ability to prioritise tasks and responsibilities whilst ensuring assessments are completed within an agreed timeframe.

6. To work in partnership to improve collaboration and coordination to achieve the best possible outcomes for Carers.

7. To work as part of a team, participating in team meetings, team building, training and activities.

8. To prepare for and participate in regular supervision meetings and to seek additional supervision as required.

9. To demonstrate awareness/understanding of equal opportunities and other people’s behavioural, physical, social and welfare needs.

10. To ensure that reasonable care is taken at all times for the health, safety and welfare of yourself and other persons, and to comply with the policies and procedures relating to health and safety and safeguarding within the organisation.

11. To undertake developmental tasks and carer awareness-raising as required. This may include being involved with fund-raising however this will be discussed with the worker on a case-by-case basis.

12. To carry out any other duties which fall within the broad spirit, scope and purpose of this job description and which are commensurate with the grade of the post.

This job description reflects the major tasks to be carried out by the post holder and identifies a level of responsibility at which they will be required to work. In the interests of effective working, the major tasks may be reviewed from time to time to reflect changing needs and circumstances. Such reviews and any consequential changes will be carried out in consultation with the post holder.
