We are looking for an enthusiastic Community Mental Health Practitioner to join our friendly multi-disciplinary team.

Main duties, tasks & skills required
You will hold a caseload and undertake the role of Care Coordinator for those with varied complex mental health and social care needs. You will provide assessment and specific interventions to meet the needs of service users, their carers, and families. It is expected that you will be a flexible practitioner who enjoys new challenges in meeting the developing needs of the service and our service users.
The post holder will be expected to work proactively in forging professional working relationships with multi-agencies.
Car driver is preferrable but not essential. You will be working 9am -5pm Monday to Friday and occasional weekend or bank holidays. We are flexible regarding working patterns and working hours.
