Hours – 18.75 hours per week
Salary – Ren Scale 8-13 £26184.00 - £31,053.00 (Pro Rata, Depending on experience) Actual salary £13,092.00 - £15,526.50
Permanent – Funding secure until March 2028 – Extension likely

Successful candidates will deliver low level social care and well-being support for people aged 16+, living with, at risk of and/or affected by HIV & their loved ones. Providing comprehensive case management for people living with HIV. Supporting with practical matters such as medication management, housing and benefits, emotional challenges such as discrimination, Stigma, health issues such as HIV related long term health conditions, contraception or accessing general practice. HIV Prevention in communities through awareness and attendance at events.
Successful candidates will work remotely across all areas of Lancashire with an expectation to attend head office in Blackpool a minimum of twice a month.
Main Duties and Key Responsibilities
- Offer screening, referral, advice, information, advocacy and support, to service users accessing or seeking to access the service, and to provide a range of interventions dependent on the needs of the service and the service user.
- Identify, inform and prioritise the immediate and potential ongoing needs of service users, providing appropriate advice, information and brief interventions, signposting, low level social care and wellbeing support, as well as facilitating access to other local services as required.
- Manage a wide range of service users, by caseload, to achieve targets.
- Ensure service users receive a personalised Wellbeing offer that meets individual needs/levels of complexity.
- Develop personalised self-management plans that inspire hope and demonstrate possibilities
- Develop care and support plans collaboratively with service users
- Design and deliver group activities that engage and inspire
- Advocate fearlessly for service users, challenging discrimination and stigma
- Design and facilitate empowering group sessions/activities that engage and inspire
- To provide assertive outreach to REVOLUTION UK service users, offering safer sex resources, specifically people living with and/or affected by HIV or engaging in risky sexual behaviours.
- To deliver dry blood spot and 60 second rapid testing for HIV and Syphilis, as well as full screening services in the community where funding allows.
- To coordinate and manage community testing events and local awareness campaigns.
Person Specification
Physical Requirement/Qualifications
- Educated to a minimum of GCSE level with Maths and English grade 4/C or above
- Relevant qualification in health, social care, public health, or equivalent experience
- Safeguarding training (or willingness to complete)
- Current driving licence and access to own vehicle
- Lived or work experience in supporting vulnerable adults
Experience
- Knowledge and understanding of current sexual health and harm reduction related issues in both England and in Lancashire.
Knowledge
- Knowledge and or Lived experience of communities that experience poor sexual health outcomes.
- Knowledge of HIV and HIV related conditions
Skills
- Excellent listening, communication, organisational, case working, and administrative/IT skills.
- Ability to operate Microsoft Office Programs, including Outlook, Word, Excel and Powerpoint.
- Social media, and digital skills including the use of Applications and smart devices.
- Previous training or experience in a coaching role
Desirable
- Experience and or knowledge of counselling skills
- Previous training or experience in a coaching role
- Good local knowledge
- An empathetic communicator who builds trust and rapport easily.
