Overview
We are seeking a dedicated and compassionate CHC Nurse Assessor to join our healthcare team. The successful candidate will play a vital role in evaluating patients' health needs to determine eligibility for Continuing Healthcare funding. This position offers an opportunity to utilise your clinical expertise in a dynamic environment, contributing to patient-centred care and ensuring fair assessment processes. The role is fully paid and requires a professional, organised approach to deliver accurate and timely assessments.

Duties

Conduct comprehensive assessments of patients’ health status, focusing on physical, psychological, and social care needs
Review medical records and liaise with healthcare professionals to gather relevant information
Evaluate patients’ eligibility for Continuing Healthcare funding based on established criteria
Prepare detailed reports documenting assessment findings and recommendations
Communicate assessment outcomes clearly and sensitively to patients and their families
Maintain accurate, up-to-date documentation in accordance with organisational policies and data protection regulations
Participate in multidisciplinary team meetings to discuss complex cases and share insights
Ensure compliance with all relevant clinical guidelines, policies, and legal requirements during assessments

Experience

Proven experience in patient care within a clinical setting, demonstrating strong clinical judgement
Knowledge of human anatomy and the ability to apply this understanding during assessments
Previous experience conducting health assessments or working within a healthcare evaluation environment is desirable but not essential
Excellent organisational skills with the ability to manage multiple cases efficiently
Compassionate communication skills, capable of engaging with patients from diverse backgrounds
Ability to work independently whilst collaborating effectively within a multidisciplinary team
