Are you an organised, compassionate, and proactive care professional looking to take the next step in your career? We're seeking a passionate Care Coordinator to join our growing home care team, delivering exceptional care and support to clients across York & East Yorkshire.

About the Role

As a Care Coordinator, you'll play a key role in ensuring the smooth day-to-day delivery of high-quality domiciliary care services. You'll be responsible for coordinating care packages, supporting our care team, maintaining compliance, and ensuring every client receives outstanding person-centred care. Working closely with the Registered Manager, you'll help drive quality, efficiency, and continuous improvement across the service.

Key Responsibilities

Coordinate and schedule care visits to ensure efficient and effective service delivery.
Support and supervise care staff, providing guidance and day-to-day assistance.
Ensure care plans and risk assessments are accurate, up to date, and regularly reviewed.
Monitor service delivery to ensure compliance with CQC regulations and company quality standards.
Liaise with clients, families, healthcare professionals, and local authorities to ensure care needs are met.
Respond to changes in client needs and coordinate appropriate adjustments to care packages.
Assist with staff onboarding, training coordination, spot checks, and competency observations.
Support the Registered Manager with audits, compliance, and the day-to-day operation of the service.
Contribute to service development, quality improvement initiatives, and maintaining excellent client satisfaction.

Essential Requirements

Right to work in the UK.
Minimum 2–3 years' experience in a senior care or care coordination role within domiciliary care.
Excellent organisational and time management skills, with the ability to prioritise workloads.
Good understanding of CQC regulations, safeguarding, and person-centred care.
Experience creating and reviewing care plans and coordinating care services.
Full UK driving licence and access to a vehicle.
Flexible approach to working hours, including participation in the on-call rota and occasional weekends where required.
Excellent communication skills, both written and verbal.
Highly motivated with a desire to progress into a Deputy Manager role.

Desirable

Experience working with York, East Riding of Yorkshire Council (ERYC), and Hull City Council.
Knowledge of local authority commissioning processes and funding pathways.
Experience using care planning and rostering systems.
A proactive team player with excellent problem-solving skills and a commitment to delivering outstanding care.
