Full job description

The Role

You will play a key role in delivering safe, effective and patient-centred care to our diverse patient population, while also providing clinical leadership to our nursing team.

You will work alongside another Practice Nurse, a Diabetes Specialist Nurse and three Healthcare Assistants, helping to coordinate the nursing team, promote high clinical standards and ensure patients receive consistent, high-quality care.

Alongside your clinical responsibilities, you will take a lead role in infection prevention and control, supporting best practice across the practice and helping to ensure we maintain robust standards in line with current guidance.

Your clinical work will include a varied mix of long-term condition management, immunisations, acute care, wound care, contraception, cervical screening and minor procedures.

Hours: Full-time, Monday to Friday between 08:00–18:30, with flexibility to cover one late evening per week. You will also work one Saturday clinic per month (approximately 2 hours); when working the Saturday clinic, you will finish earlier on the Friday.

Salary: £50,000 per annum, depending on experience
Reports to: Clinically – GP Partner | Administratively – HR Manager

Clinical & Leadership Responsibilities

Provide high-quality clinical care across a broad range of primary care services.
Provide day-to-day clinical leadership and support to the nursing team, working closely with the Practice Nurse, Diabetes Specialist Nurse and HCAs.
Help coordinate nursing services to ensure clinics are well organised, effective and patient-centred.
Support colleagues with clinical decision-making, escalation and complex patient care where appropriate.
Contribute to the development of the nursing team through mentoring, knowledge sharing and supporting good clinical practice.
Take responsibility for infection prevention and control, ensuring policies, procedures and standards are maintained across the practice.
Support infection control audits, identify areas for improvement and implement changes where required.
Run clinics for long-term conditions and support ongoing disease management.
Deliver immunisations and vaccination programmes.
Provide assessment and treatment for acute presentations.
Undertake wound care, contraception, cytology and minor procedures.
Deliver care in line with current NICE/NSF guidance and best practice.
Maintain accurate, high-quality clinical records using SNOMED.
Participate in clinical audits, QOF/LCS work and quality improvement initiatives.
Work collaboratively with the wider practice, PCN colleagues and external services.
Maintain your professional development, CPD and revalidation requirements.

What We’re Looking For

We are seeking an experienced Practice Nurse who is confident clinically and is ready to take on a leadership role within primary care.

Essential:

Registered General Nurse with current NMC registration.
Nursing degree or diploma.
Specialist qualification or significant experience in primary care and/or long-term condition management.
Proven experience working in primary care nursing.
Experience in long-term conditions, immunisations, acute care, QOF/LCS delivery, audits and infection prevention and control.
Strong communication, organisation and leadership skills.

Desirable:

Independent or supplementary prescribing qualification.
Relevant postgraduate or specialist qualifications.
Previous experience of leading or supervising a nursing or clinical team.
Experience of taking responsibility for infection prevention and control.
