We are excited to announce a new opportunity within our Hampshire and Isle of Wight All Age Continuing Care Personalisation service. We are looking for a person who is passionate about client care and improving life opportunities who share our behaviours of being person centred, honest, courageous, innovative, and collaborative.
As a Personalisation and Personal Health Budget Practitioner, you will be responsible for maximising the take-up of Personal Health Budgets and acting as a source of expertise and advice for relevant members of the multi-disciplinary teams, patients and members of the public in order to support the Personal Health Budget Process. The role involves working in partnership with a clinicians to implement Personal Health Budgets, including supporting and advising on the development of care and support plans, monitoring and managing client financial information and payments and providing advice and guidance to clients to empower people with complex health needs live their best lives.
Whilst experience in Personal Health Budgets and accounting would be helpful, if you have the right skills and attitude, we can support you to develop the specific skills required. You will be part of a wider personalisation and All Age team who provide a wide range of expertise and support.

Main duties, tasks & skills required
The post holder will act as a source of expertise and advice for relevant members of the multi-disciplinary teams, patients and members of the public in order to support the Personal Health Budget Process
To support adults and children to access healthcare and support through Personal Health Budgets
Maximise the take-up of Personal Health Budgets (prioritising All Age Continuing Care clients) throughout health and social care
Working in partnership with and through clinicians at Place to implement Personal Health Budgets, including supporting and advising on the development of care and support plans to meet identified health and care needs/health and wellbeing outcomes
Supporting and educating case managers on the calculation of indicative costs, providing recommendations and financial analysis to All Age Continuing Care and (as required) Local Authority members/the Personal Health Budget Panel
Supporting finance to monitor and manage client financial information to include recovering unspent and misspent funds
Providing advice and guidance to clients and clinicians as and when required, acting as an advocate to ensure the best possible outcomes can be achieved

The All-Age Continuing Care Team supports over 7,500 clients through a range of funding arrangements including Continuing Healthcare, Section 117 aftercare, specialist placements, Children's Continuing Care and other joint funding and oversight of a budget of 392M. Our roles are community based with flexible working from a local office and home and is a key role to support people to live well with complex health and primary health needs. Post holders will be employed by Hampshire and Isle of Wight Healthcare NHS Foundation Trust, working in partnership with NHS Hampshire and Isle of Wight Integrated Care Board.
