Full job description

JOB TITLE: Care Navigator

REPORTS TO: Operations Manager & Team Leader

HOURS: 28 hours per week

Monday: 8am – 7pm (10 hours)

Tuesday: Day off

Wednesday 1pm – 7pm (6 hours)

Thursday 1pm – 7pm (6 hours)

Friday 1pm – 7pm (6 hours)

Pay: Dependent on experience. On the job training can be provided at the surgery.

Job summary:

The main responsibility of the role is:

· Working on the front desk at the surgery, handling patients in-person queries.

· Answering the phones, handling patient queries via the phone.

· Process and respond to online admin requests and queries submitted to the practice.

· Working alongside and supporting the duty doctor to arrange & book appointments along with processing urgent admin tasks such as booking on the day home visits.

· Process incoming emails & post into the surgery.

· Undertake clinical admin such as new patient registrations, processing prescriptions, raising blood test forms, booking patients for immunisation & long-term condition review appointments.

Our expectation is in periods with high levels of leave; is the post-holder would support the wider Non-Clinical Team at the Surgery providing cross-cover for the scanning & processing of patient documentation/letters, and some less complex Secretarial referral duties.

Candidate requirements:

Has a keen eye for detail
To work in accordance with written protocols at the surgery.
Have a high level of IT literacy.
Ability to adapt to an ever-changing demand on primary care, to deliver modern general practice.
Is keen to develop themselves and learn new skills.

· SystmOne knowledge

· AccuRx knowledge

· Primary Care experience

Health & safety:

The post-holder will assist in promoting and maintaining their own and others’ health, safety and security as defined in the practice health & safety policy, the practice health & safety manual, and the practice infection control policy and published procedures. This will include:

Using personal security systems within the workplace according to practice guidelines
Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and free from hazards
Actively reporting of health and safety hazards and infection hazards immediately when recognised
Keeping own work areas and general/patient areas generally clean, assisting in the maintenance of general standards of cleanliness consistent with the scope of the job holder’s role
Reporting potential risks identified
Demonstrate due regard for safeguarding and promoting the welfare of children.
