Surrey and Borders Partnership NHS Foundation Trust is looking for someone who is passionate about ensuring a high quality holistic approach to the care and support provided to clients in the community who have been identified as in need of community nurse support to enable them to exercise choice over their own lives.

Working Pattern: Monday to Friday from 9 am to 5 Pm (3 months)

Location: 18 Mole Business Park, Randalls Road, Leatherhead, Surrey, KT22 7AD

To provide high-quality, holistic, person-centred care to service users in the community.

To lead the clinical management of a defined caseload, ensuring care is effectively planned, implemented, and evaluated.

To coordinate and deliver evidence-based interventions, including CBT-informed therapeutic support.

To work collaboratively within the multidisciplinary team (MDT) and with external agencies.

To undertake comprehensive assessments, including risk assessments, and respond appropriately to changes in clinical presentation.

To monitor and support both the physical and mental health needs of service users, including undertaking clinical observations.

To promote recovery, independence, and choice, enabling service users to exercise control over their lives.

To ensure safeguarding responsibilities are met and risks are managed in line with policies and procedures.

To maintain accurate, timely, and legally compliant clinical records.

To communicate effectively with service users, carers, families, and professionals.

To contribute to service development, quality improvement, and audit processes.

To support and contribute to staff development, supervision, and learning.

To ensure compliance with professional standards, including the NMC Code, and Trust policies.

To promote a positive image of the team and organisation at all times.

Surrey and Borders Partnership NHS Foundation Trust is the leading provider of health and social care services for people of all ages with mental ill-health and learning disabilities in Surrey and North East Hampshire. We provide support to a population of 1.3 million across 140 services.

We actively seek to engage people who use our services and our communities in improving the mental wellbeing of the local population. We work closely with other NHS and voluntary sector organisations who provide services and support people who use services and carers.

This role is working within i-access, a successful, countywide drug and alcohol treatment service for Surrey residents delivering tier 2, 3 and 4 substance misuse interventions across 3 main bases and 25 satellite locations. i-access is run collaboratively by SABP, PH / SCC and Via under a cooperative agreement which is one of partnership and transparency and contributes to Surrey Heartlands wider strategic ambitions.

Clinical Responsibilities

To lead the clinical management of care packages for a defined group of service users.

To assess, plan, implement and evaluate care in line with evidence-based practice and Trust policies.

To coordinate and deliver person-centred care, using advanced clinical decision-making where appropriate.

To deliver therapeutic interventions, including individual and group work informed by CBT principles.

To provide holistic care addressing physical, psychological, and social needs.

To undertake and record physical observations (e.g. blood pressure, pulse, temperature, blood glucose, urinalysis) and respond appropriately to any concerns.

To monitor changes in service users’ health and wellbeing and escalate concerns as required.

To maintain professional boundaries while offering therapeutic support.

To respect and promote service users’ diversity, dignity, values, and beliefs.

Communication

To maintain accurate, contemporaneous clinical records in accordance with Trust policy and professional standards.

To communicate effectively with service users, carers, families, and professionals.

To adapt communication methods to meet individual needs and overcome barriers.

To ensure valid consent is obtained prior to interventions.

To ensure all communication complies with relevant legislation, policies, and procedures.

Quality and Service Improvement

To contribute to the development, implementation, and monitoring of service standards.

To participate in audit, research, and data collection activities as required.

To identify areas for service improvement and contribute to change initiatives.

To promote shared decision-making and service user involvement in care planning.

To raise concerns and report incidents in line with Trust procedures.

To represent the team and Trust in a professional manner at all times.

Risk and Safeguarding

To undertake comprehensive risk assessments and implement risk management plans.

To recognise and respond appropriately to crisis situations.

To participate in the Care Programme Approach (CPA) process.

To identify safeguarding concerns and act in accordance with local and national safeguarding policies.

To promote the safety and wellbeing of service users, staff, and the public.

To support service users in line with their agreed leave status and risk management plans.

Professional Development

To participate in clinical supervision and annual appraisal.

To identify personal development needs and undertake relevant training.

To maintain professional registration and adhere to the NMC Code.

To complete all mandatory training requirements.

To support the learning and development of students and new staff.

Leadership and Management

To support the recruitment, supervision, and development of staff.

To contribute to maintaining safe staffing levels and skill mix.
