About Wisteria Care Agency Ltd

Wisteria Care Agency Ltd is a specialist care provider delivering high-quality, person-centred support to adults living in their own homes and supported living environments. Our aim is to empower people to live as independently as possible whilst receiving safe, compassionate and responsive care tailored to their individual needs.

Due to our continued growth, we are looking to recruit an experienced Care Coordinator to join our management team.

About the Role

The Care Coordinator will play a vital role in ensuring the effective coordination of care services, supporting both service users and care staff to achieve the highest standards of care.

You will be responsible for coordinating care packages, producing staff rotas, maintaining continuity of care, responding to changes in service users' needs, liaising with families and healthcare professionals, and ensuring compliance with CQC regulations and company policies.

This is a fast-paced role requiring excellent organisational, communication and problem-solving skills.

Essential Requirements

Minimum of 3 years' experience as a Care Coordinator or in a similar coordination role within the domiciliary care, supported living or home care sector.
Excellent understanding of CQC Fundamental Standards and current health and social care legislation.
Experience using electronic care planning and rostering systems (e.g. One Touch, PASS, Birdie or similar).
Strong organisational and time management skills.
Excellent verbal and written communication skills.
Ability to work under pressure and prioritise workloads effectively.
Good IT skills, including Microsoft Office.
Full UK driving licence and access to a vehicle (preferred)

Pay: £25,000.00-£27,000.00 per year
