Job Overview

We are seeking a dedicated and energetic Senior Supported Housing Worker to join our dynamic team. In this vital role, you will provide compassionate support and guidance to individuals living in supported housing environments that have come from a rough sleeping background, ensuring they receive the care and assistance needed to thrive. This is not your average care role. Your leadership will empower residents to achieve their personal goals to secure permanent housing, while promoting a safe, respectful, and engaging community. This position offers an exciting opportunity to make a meaningful difference in people's lives through proactive support, effective communication, and collaborative teamwork. This is not a personal care role.

Responsibilities

Develop and implement personalized care plans tailored to each resident’s unique needs, goals, and circumstances.
Support residents with daily activities, including personal care, household tasks, and social engagement.
Foster open and respectful communication with residents, their families, and external service providers to ensure coordinated care.
Monitor residents’ well-being regularly, identifying any concerns or changes that require additional support or intervention.
Encourage independence by empowering residents to participate actively in their own care and community life.
Maintain accurate records of support activities, progress notes, and incident reports using IT systems efficiently.
Drive community engagement initiatives that promote social inclusion and positive interactions among residents.

Requirements

Proven experience working with children or vulnerable adults in supported housing
Strong communication skills with the ability to listen actively and convey information clearly.
Ability to develop and follow detailed care plans tailored to individual needs.
Valid driving license and willingness to use a vehicle for work-related travel when necessary.
Competence in using IT systems for record-keeping, reporting, and communication purposes.
Knowledge of safeguarding procedures, health & safety protocols, and confidentiality standards.
Flexibility to work varied shifts including evenings, weekends, or on-call duties if required.
