Rhondda Cynon Taf Council is pleased to offer an exciting Care & Support Practitioner position within the Care Management Team.

We want to ensure our services help people achieve their full potential. That means working with people to understand what matters to them and how we can help.

We are looking for a practitioner who is positive and enthusiastic and can work with individuals, families, and carers in line with the requirements and principles of the Social Services and Wellbeing Act (Wales) 2014, and the Mental Capacity Act 2005. We are seeking a practitioner who is able to put the person at the heart of their work.

The successful candidate will undertake person centred assessments, draw up care plans with individuals and their carers to ensure care needs are met while reviewing care and support plans. Working with individuals from the age of 18 years old who have care and support needs. You will also be undertaking Section 126 enquires in line with safeguarding where you will feed back outcomes. You will need to balance risk against self-determination, and uphold the rights of the individual, with the ability to deliver quality services.

The ability to appropriately manage risk is essential. Carrying a varied case load in a fast-paced environment, ensuring completion of assessment are met within timescales.

Working as part of a team, but able to also work autonomously, you will have excellent verbal and written skills and be computer literate. You will need to be organised, self-motivated and be able to prioritise competing demands on your time as well as being creative and resourceful and have a strong belief in empowering and enabling adults towards independence.

Professional development is encouraged and expected in RCT, and the Council has an excellent training department with varied opportunities to participate in training in line with the role of a care and support practitioner. Regular professional supervision is provided, and you will also have opportunity to become involved in peer group supervision.

It would be desirable to have previous experience of working in a care management role or social care within Adult Services, knowledge and understanding of working within adult health / social care and be conversant with all current legislation in Wales.
