About us

Key Responsibilities:

To ensure that the service provides a safe, client focused environment that utilises a holistic approach to support.
Provide practical and emotional support to clients; giving guidance on issues as they arise.
Provide a holistic approach in supporting clients to achieve the goals outlined in their support plan.
Effectively engage and empower clients to review choices and behaviours and make positive changes to their lifestyle; working in partnership with other services as appropriate.
To support the client to maintain their tenancy and to develop the skills required to live independently in the future.
Work with the housing management team to assist the client with move on at the point that they are ready for service exit.
To assess needs and risk in partnership with the client; ensuring that support and risk management plans are robust.
To regularly review support and risk management plans ensuring that the support aims are in line with the clients needs and wishes, and that the level of support provided is suitable.
Work in partnership with the housing management team to ensure best possible outcomes for customers.
Ensure that all required documentation is accurately maintained and up to date.
Ensure that safeguarding’s, incidents, accidents and near misses are reported and responded to appropriately.
Support the Project Manager in ensuring that the service meets it’s key performance indicators, addressing any barriers appropriately.
Provide data as required by the Support Project Manager.
Report all maintenance and health and safety concerns to the Housing Management team and support clients to do so.
Support and encourage clients to attend house meetings..
Assist your manager in responding to internal / external audit ensuring that any actions are completed in a timely manner.
DBS checks are required for roles where you will be working with or have lone contact with Vulnerable Adults

EDUCATION, TRAINING, SKILLS AND WORK EXPERIENCE

Understanding of safeguarding practice
Passionate about working with vulnerable people
Good level of literacy, numeracy and I.T skills.
Good verbal and written communication skills.
