Full job description

Location: WiEducate, 8 Cheyne Walk, Northampton (travel across North and West Northamptonshire)
Reports to: Directors
Contract: Part Time- Permanent (4days)
Salary: £34 000

Requirements: Business insurance/access to a car (expenses will be paid)

WiEducate offers alternate education for 16-24 years who have been unable to access other education provision in the area. WiEducate works 1:1 and in small groups to support our young people to achieve their aspirations and successfully transition into adulthood.

Purpose of the Role

The Transition Lead will coordinate and oversee all elements of learner transition within WiEducate. This includes conducting initial assessments, supporting transition into college, overseeing interim education packages, and planning successful progression pathways on exit. 

The postholder will ensure that all learners experience a structured, supportive, and person-centred transition process that meets their educational, social, and emotional needs. 

Key Responsibilities

1. Consultations & Referrals

Act as the lead contact for all new learner initial assessments.
Liaise with, schools, parents/carers, and professionals to gather information and create support plans.
Create risk assessments, learner profiles and supporting documentation.
Lead initial assessment meetings, ensuring clear communication about provision, expectations, and outcomes.
Maintain accurate records of enquiries, consultations, and decisions.

2. Initial Assessment

Deliver a comprehensive initial assessments for prospective learners.
Assess suitability, communication, independence, behaviour, and support needs.
Identify appropriate pathways, curriculum offers, and support packages.
Produce detailed assessment reports to inform placement decisions and planning.
Ensure assessments are person-centred and inclusive of learner voice.

3. Transition into College

Plan and coordinate individualised transition programmes for new learners.
Develop transition timetables, phased starts, and bespoke support strategies.
Work closely with teaching, therapy, and pastoral teams to ensure readiness.
Build relationships with learners and families to reduce anxiety and ensure smooth transitions.
Ensure all safeguarding, medical, and risk information is effectively communicated before start.

4. Interim Education Provision

Oversee interim education packages for learners awaiting placement or transitioning from other settings.
Coordinate interim learning, or alternative provision as required.
Monitor engagement, progress, and wellbeing during interim periods.
Ensure continuity of education in line with EHCP outcomes and statutory responsibilities.
Liaise with external providers where necessary.

5. Ongoing Transition Planning (Internal)

Monitor learner progress during the early stages of initial assessment.
Adjust transition plans based on individual needs and responsiveness.
Support staff with strategies for integration and inclusion.
Lead review meetings related to transition progress.

6. Transition Out of College

Lead on exit planning, including progression to employment, supported internships, further education, or independent living.
Work with careers advisors, employers, and external agencies to secure appropriate placements.
Contribute to EHCP annual reviews and transition reviews.
Ensure learners are prepared with the skills needed for their next stage.
Maintain destination tracking data.

7. Partnership & Multi‑Agency Working

Build strong partnerships with Local Authorities, schools, health professionals, and social care teams.
Represent the provision at external meetings and transition forums.
Ensure compliance with SEND Code of Practice and statutory guidance

8. Safeguarding & Compliance

Ensure all processes adhere to safeguarding, health and safety, and data protection policies.
Identify and escalate any safeguarding concerns.
Maintain accurate and confidential records.

Person Specification

Essential

Experience working with young people aged 16–25, particularly those with social, emotional and mental health needs
Strong understanding of transition processes within education or specialist provision.
Experience of conducting assessments and writing reports.
Excellent organisational and case management skills.
Strong communication and interpersonal skills, with the ability to engage families and professionals.
Knowledge of safeguarding and SEND legislation (e.g. SEND Code of Practice).
Ability to manage multiple cases and prioritise workload effectively.

Desirable

Teaching qualification, SEND qualification, or relevant professional background (e.g. social work, therapy, careers guidance).
Experience working in post‑16 or specialist college settings.
Knowledge of supported internships, employment pathways, and independent living programmes.
Experience of working with Local Authorities and commissioning processes.
