Job: Swyddog Adolygu ac Asesu Dyletswydd / Duty Review & Assessment Officer

Salary: £34,434.00 - To: £36,363.00

Job Scale: Gradd 6 / Grade 6

Job Hours: 37 Awr / 37 Hours

Location: Parc Iechyd Kier Hardie / Kier Hardie Health Park

Speciality:

Provide appropriate information and/or undertake a proportionate assessment and promote a culture of independence and positive outcomes when signposting or commissioning services.

To be part of a busy Duty Team, To carryout Assessments in line with the Social Service and Well-being Act 2014.
You will be covering the duty Rota on designated days, you will be expected to cover Absences if required.

Engaging in a structured conversation with those making initial enquiry of the service and evaluating any information given;

Giving information, advice/and or sign- posting to other services;
Ensure that information about advocacy services is provided.

Accurately and fully recording referral information in accordance with departmental procedures;

Identify any need for advocacy to support the service user to participate in the IAA service proportionate assessment.

Identify any safeguarding concerns and take appropriate actions.

To carry out community care assessments ensuring service users are having the required service to meet their needs, this could range from a package of care, day services, respite and linking in with 3rd sector and signposting to other agencies where appropriate.

To undertake all 126 enquires and undertake a visit if deemed necessary.

Agreeing a range of solutions to meet identified outcome with adults and carers by providing short term respite for service users in a residential setting.

Where appropriate approach and undertake Mental Capacity assessments to fulfil the requirements of the MCA code of practice.

Undertake reviews in accordance with departmental policies.
