Full job description

Hours per week: 36 hours (12 hours shifts)

Reports to: Service Manager

Location: Burton on Trent

Shift timings: 7am to 7pm

Benefits:

Enhanced Bank Holiday pay – paid at 1.5 times your normal hourly rate

Job Purpose

To be responsible for the provision of support for young people 16 -18-year-old

Main Duties & Responsibilities

1. To assist the young people in their independent living journey.

Support to young people as directed in line with support plans
Conduct welcome and induction for young people moving into the property
Update of all paperwork, risk assessments, support plans
Provide ongoing support and monitoring of the young person
Assist in the management of all health issues, physical, mental and emotional needs
Preparation of weekly reports to the social workers
Arrange appointments with various partner agencies
Maintaining regular contact with young people
Encourage young people to engage with groups in the community
Provide structure for the young person e.g., register with a college or educational settings
To accurately record, create, maintain and update records in respect of young people
To prepare for and attend meetings as required
To effectively use IT packages
To work effectively and cooperatively across multi-disciplinary teams.
To observe and maintain strict confidentiality in respect of the young people and Company
To pro-actively assist in the development of policies and procedures of the Company
To report any housing issues to the maintenance team
Attend mandatory team meetings, training and supervisions on a regular basis as requested by management
Maintain cleanliness of office space and communal areas, ensuring they are well-kept and hygienic for all residents
Perform cleaning duties to prepare rooms for new occupants, including after a young person moves out, ensuring the space is clean, welcoming, and ready for the next resident.

General Accountabilities

Ensuring the highest standards of quality are achieved in all duties carried out.
Communicating effectively within the team, with other teams and across the company.
Supporting the culture of outreach services cross the area in our homes.

Health and Safety

The outreach worker is required to carry out the duties in accordance with the company Health and Safety policies and procedures.

The outreach worker is required to have due regard to equal opportunities at all times and to work in a fair and reasonable manner towards all people, ensuring service standards are maintained for all cultures.

Confidentiality

The outreach worker is required to observe and maintain strict confidentiality in respect of al young people and company information.

Other Duties

The duties and responsibilities in this role profile are not exhaustive. The post holder may be required to undertake other duties that may be required from time to time within the general scope of the post. Any such duties should not substantially change the general character of the post. Duties and responsibilities outside the general scope of this post will be mutually agreed with the post holder.

Qualifications/Requirements:

QCF Level 3 in Children and Young People’s Workforce or equivalent or working towards it (Essential)
Enhanced DBS record (Essential)
Current full driving license with business Insurance - Cost of business insurance to be covered by the employee (Essential)
Willingness to undertake further training (Essential)

Experience:

In a relative social care environment (Essential)
Experience of working in a supported accommodation (Desirable)
In dealing with individuals from ethnic minority groups (Desirable)
In dealing with vulnerable people from a variety of backgrounds (Desirable)
Frequent use of computerised admin systems in busy working environment (Essential)

Aptitude:

Good written, verbal, and communication skills (Essential)
Able to work to tight deadlines (Essential)
Confident on a computer and with Microsoft packages (Essential)
Able to deal with conflict situations in a calm yet firm manner (Essential)
Able to make quality decisions under pressure (Essential)
