Tylorstown Group Practice, Ferndale Road, Tylorstown, Mid Glamorgan CF43 3HB.

Practice Administrator / Care Navigator
25 hours per week
Salary: Depending on experience

We are a friendly GP practice in the Rhondda area looking to recruit a Practice Administrator / Care Navigator to join our busy and supportive team.

This is a combined patient‑facing and administrative role, helping patients access the right care at the right time, while supporting the smooth running of the practice.

The role includes:

Care navigation and signposting patients to appropriate services
Reception and telephone support
General practice administration and data input
Management of incoming correspondence, including scanning, workflow and document handling
Summarising and read coding of patient records
Supporting private workload processes (e.g. medical reports, forms, and non-NHS work)
Working closely with clinicians and the wider practice team

We are looking for someone who is:

Organised, reliable, and confident communicating with patients
Able to work calmly in a fast‑paced environment
IT‑literate with good attention to detail
General practice administration
Summarising and read coding patient records
Managing scanning and document workflows
Handling private/non‑NHS workload processes
