Job Summary
We are seeking a highly organised, proactive, and patient-focused individual to join our GP practice team in the newly developed role of Patient Care Coordinator. This is a central and evolving position that reflects the changing landscape of primary care, moving beyond traditional secretarial duties to encompass a broader range of responsibilities.
The successful candidate will act as a key point of contact for patients navigating the healthcare system, managing referral pathways, following up on outstanding actions, and responding to clinical correspondence and administration requests. You will work closely with GPs, clinical staff, and external healthcare organisations to ensure patients receive timely, coordinted, and effective care.
Applicants must have at least 1 year of experience in a GP practice or primary care setting. This requirement is essential for the role.
The ideal candidate will demonstrate excellent organisational and communication skills, strong attention to detail, a confident and compassionate approach and a solid understanding of NHS referral pathways and primary care workflows.
Key Responsibilities
Patient Coordination
- Helping patients access the right service at the right time through the right route
- Proactively follow up rejected and outstanding referrals, ensuring timely progress and communicating outcomes to patients and clinical staff
- Support patients with queries related to their care journey, responding with clarity and sensitivity
Clinical Correspondence & Administrative Support
- Manage incoming clinical messages, tasks, and administrative requests, acting on these in a timely and accurate manner
- Process requests for medical reports (e.g. insurance, employment, DVLA, solicitor reports), coordinating with GPs and ensuring appropriate completion
- Prepare and manage outgoing correspondence including referral letters and clinical summaries
- Handle incoming correspondence from hospitals, community services, and other providers, triaging and actioning appropriately
Records & Data Management
- Maintain accurate and up-to-date patient records in line with confidentiality and data protection requirements
- Support coding, data quality, and accurate recording of clinical information within the practice system (EMIS)
- Assist with QOF (Quality and Outcomes Framework) processes, including running searches and recalls, monitoring patient registers, and supporting data accuracy
General Practice Administration
- Provide administrative support to GPs and the wider clinical team as required
- Liaise professionally with patients, hospitals, and external healthcare organisations
- Undertake general office duties including scanning, document management, and inbox management
Requirements
Essential
- At least 1 year of experience working in a GP practice or primary care setting
- Familiarity with NHS referral pathways, systems, and primary care workflows
- Strong knowledge of medical terminology
- Proficiency in Microsoft Office and clinical systems (e.g. EMIS and Docman)
- Excellent written and verbal communication skills
- High level of accuracy and attention to detail
- Strong organisational and time management skills
- Understanding of data protection legislation and patient confidentiality requirements
- Ability to work independently and as part of a multidisciplinary team
- Confident, compassionate, and professional approach to patient contact
Desirable
- Understanding of QOF processes and primary care targets
- Experience handling medical report requests
Hours: 35 per week
- Monday: 9am to 4pm (30 minute unpaid break)
- Tuesday: 9am to 6:30pm (1 hour unpaid break)
- Wednesday: 10:30am to 6:30pm (30 minute unpaid break)
- Thursday: 12:30pm to 4:30pm
- Friday: 10am to 6:30pm (30 minute unpaid break)
- Satudays: 9am to 12pm (every six weeks)
