Full job description

The Assistant Practitioner has additional responsibilities over and above the role of Carer – including management support, quality assurance, and peer support to ensure smooth day to day running of the unit. The Assistant Practitioner is responsible for ensuring that the standard of care across all shifts is consistently high: in line with company procedures and best practice. The Assistant Practitioner will coach and mentor Carers and must take charge of the Shift when asked to do so by the Nurse or the Registered Manager. The Assistant Practitioner is accountable to the Lead Nurse, and reports day to day to the Nurse in Charge of the shift.

Minimum qualifications

NVQ3 in Health & Social Care

Diploma Senior Healthcare Support Worker

Assistant Practitioner Qualification

Main duties – in addition to those of a Care Assistant

1. To ensure that work is conducted according to CQC requirements, the Social Care Institute of Excellence and relevant professional guidelines.

2. To complete specific tasks as assessed as being competent to do so including but not limited to dressings, managing catheters, and managing PEGs

3. To practice safe control, administration and storage of Service Users’ medication as prescribed by the General Practitioner and in line with MAR charts and medication care plans, in line with individual competencies only

4. Conduct routine observations (e.g. blood pressure, temperature) under delegation from a Nurse

5. To ensure all records and documents are updated and evaluated as needed under delegation from a Nurse

6. To recognise deteriorations in health, emotional state, skin integrity and report appropriately.

7. To perform basic life support using appropriate resuscitation techniques

8. To assist the team with the management of high dependency Service Users or those displaying Behaviour that Challenges.

9. To support across the site in an emergency situation where other senior staff are not available

10. To provide coaching and leadership for Carers and others, so that they develop their competence and skills which enable them to become competent practitioners and be able to support staff

11. To use initiative to ensure smooth running of the shift, as instructed by the Nurse in Charge

12. To work with the Nurse in Charge to ensure effective communication on shift and across the Home

13. To support the care staff team to utilise skills appropriately to meet resident needs, including fair distribution across different areas of the home

14. To take appropriate action in response to incidents or emergencies.

15. To ensure that communication between the Service User, their families, and the Home is appropriate and effective

16. To ensure that all staff on shift are aware of any major changes in Service User dependency, preference or any other updates internally that all staff need to be aware of

17. To assist with other duties as required
