Job Advert: Care Navigator Assistant Team Leader

Hours: Full Time (37 hours per week)
Location: Tonbridge Medical Group
Contract: Permanent

About the Role

We are looking for an enthusiastic, organised and compassionate Care Navigator Assistant Team Leader to support the Care Navigator Team Leader and the Team.

This is an exciting opportunity for someone with excellent leadership and communication skills who is passionate about delivering outstanding patient care and ensuring patients receive the right support, from the right person, at the right time.

Working closely with the Practice Manager, Operations Manager and clinical teams, you will provide operational leadership to the Care Navigation team, ensuring high standards of patient service, effective appointment management and continuous improvement across reception and care navigation services.

Key Responsibilities

Provide day-to-day supervision and support to the Care Navigator Team Leader and team.
Act as a first point of contact for team queries and escalated patient concerns.
Support recruitment, induction, training and development of new staff.
Monitor staffing levels and assist with rota planning and sickness cover.
Ensure care navigation processes are delivered consistently and safely.
Coach team members in effective care navigation and excellent customer service.
Monitor performance against agreed standards and support quality improvement initiatives.
Promote a positive, inclusive and supportive working environment.
Work collaboratively with GPs, nurses, allied health professionals and administrative teams.
Ensure compliance with NHS policies, information governance and confidentiality requirements.
Deal with complaints.

About You

You will be a confident leader with experience working in a busy healthcare, primary care or customer service environment.

You will have:

Essential

Experience working in an NHS, GP Practice or healthcare setting.
Previous experience supervising or leading a team.
Excellent communication and interpersonal skills.
Strong organisational and problem-solving abilities.
Experience handling confidential information.
Good IT skills including Microsoft Office and clinical systems (EMIS, SystmOne or similar).
Ability to remain calm under pressure and prioritise competing demands.
Commitment to delivering excellent patient care.

Desirable

Care Navigation training.
Experience using NHS digital systems.
Knowledge of Primary Care Networks.
Understanding of NHS access standards and patient pathways.
Qualification in leadership or management (or willingness to undertake).

Person SpecificationQualifications

GCSE English and Maths (Grade C/4 or above) or equivalent.
