Job Overview

An exciting opportunity has arisen due to expansion of the team for a Band 4 Diana Children’s Specialist practitioner, Special Needs Education.

Monday to Friday, Term Time Only 08:30 15:30 - salary is pro rata

The successful candidates will work as part of the Diana Childrens Community Service team based at Ash Field Academy special school.

To work as a member of the Diana Children’s Community Service providing education and healthcare support to children/young people with complex healthcare needs, including life limiting/life threatening conditions within the school setting.
Provide healthcare and educational support in accordance with the child’s care plan/ EHCP/IEP/ statement which has been individually negotiated with the parents/carers/ SENCO
Support the qualified nursing staff/ SENCO in reviewing each individual child’s care.

This post will allow you to work within Ashfield School for pupils between the ages 4 and 19 with a range of special educational needs. You will be working with highly committed staff, governors and parents, carers all are ensuring pupils achieve the very best possible learning outcomes.

Job Description Summary (Main duties and responsibilities)

Diana Children’s Community Service provide healthcare and emotional support to children/young people with complex healthcare needs, including life limiting/life threatening conditions in a variety of settings.

Primary role of the specialist practitioner for schools worker is to provide support in the school setting based at Ash Field School. The role provides health and educational support to designated pupils within the school.

To work as part of a nurse led service caring for children/young people with life limiting illnesses and/or technological support under the supervision of registered nurses in an education setting.
Work in close collaboration with parents/carers to provide care to children and young people in accordance with the individual care plan to meet nursing, therapy and educational needs.
Contribute to the assessment of children’s and young people’s health and wellbeing alongside qualified staff.
Monitor and evaluate children’s young people’s health and care management alongside their educational targets. Ensuring all aspects are the young person’s Education and Health care plan EHCP are adhered to.
Take the lead role alongside registered nurse colleagues caring for more complex children and young people.
Provide health advice to parents/carers on a day-to-day basis responding as necessary to health needs and incidents as they arise.
Maintain and contribute to professional record ensuing confidentiality at all times.
To supervise new members of the team and support induction and preceptorship programmes.
Contribute to systems and processes, which support safe and effective care delivery including timely co-ordination of duty rota.
Report changes in children’s and young people’s condition and any issues in respect of their health and management of care to the registered nurse in a timely manner.
Ensure effective day-to-day communication in respect of children’s and young people health and care management with parents/cares.
Contribute to effective systems of clinical governance contributing to the annual clinical audit programme.
Act in accordance with the Leicestershire Medicine code for handling and storage of medicines.
Contribute to the protection of children risk in accordance with the area child protection procedures both within Leicester Partnership Trust (LPT) and School.
Meet competence requirements identified within the role and participate effectively in the personal development planning process, clinical supervision, and reflective practice. Attend training sessions relevant to post.
Extended role any other duties as reasonably requested, including attending staff meetings, parent evenings and training.

Knowledge

Understanding of child development or education to level 3 or equivalent
Understanding of equal opportunities

Experience

Experience of working with children and/ or disability.
Experience of child development.

Skills

Effective communications skills both written and verbal, including the ability to receive and provide complex /sensitive information
Desire to develop clinical skills and implement developments within the service
