Full job description

Looking for a social care role where you can use your judgement, build relationships and make a real difference to how people are supported?

We’re looking for a Trusted Assessor to work alongside adults with care and support needs and their families, carrying out strengths-based assessments and reviews and helping people find practical, personalised ways to achieve what matters to them.

This is a varied role combining assessment, problem solving, partnership working and relationship building. You’ll look beyond traditional services, exploring people’s strengths, relationships and community alongside formal support.

The role will include:

Carrying out strengths-based Care Act assessments and reviews
Developing person-centred support plans with individuals and families
Providing clear and accessible information about care, support and local opportunities
Working alongside health, social care and voluntary sector partners
Maintaining accurate records and contributing to quality and performance monitoring

About the role

Assessment and Support

Work alongside people with a learning disability and autistic people, and where appropriate their families and carers, to understand what matters to them and provide information, guidance and support to help them achieve their personal outcomes
Assess a person's strengths, needs, goals and aspirations, recognising the role of family, friends, community networks and local resources in helping them achieve the outcomes that matter to them
Use professional judgement to gather, analyse and record information accurately, making evidence-based recommendations that are proportionate and person centred
Work creatively with the person to help them develop their own person-centred support plan to maximise independence, build confidence and reduce reliance on formal services wherever appropriate
Explore community-based opportunities, natural support networks and local services that can help people achieve their goals and reduce isolation
Help people navigate the health and social care system, applying a good understanding of the Care Act 2014 and other relevant legislation to ensure people receive clear, accurate and accessible information throughout the assessment process
Carry out proportionate reviews of care and support arrangements, working with people to understand what is working well, what has changed and whether outcomes continue to be met

Advice and Coordination

Explain assessment outcomes, support planning, personal budgets, Direct Payments, financial assessments and the respective responsibilities of the Council and the individual, ensuring people understand the decisions affecting them
Ensure information is provided in a way that is accessible to the individual, using Easy Read or other communication approaches where appropriate, enabling people to make informed choices
Signpost and refer to appropriate statutory and community services
Work collaboratively with health, housing, voluntary sector and other partner organisations to coordinate support around the person
Escalate safeguarding concerns promptly

Case Management

Manage a varied caseload prioritising work effectively, responding appropriately to changing levels of risk and urgency
Maintain accurate, timely records on case management systems
Contribute to performance monitoring and quality standards
Comply with GDPR, safeguarding and organisational policies

About You

Skills, Abilities and Knowledge

Excellent communication and interpersonal skills
Ability to build rapport with people from diverse backgrounds
Ability to explain complex information clearly and accessibly
Ability to prioritise and manage a varied caseload
Ability to work collaboratively with professionals while maintaining the person's voice at the centre of decision making.
Good written communication skills, including the ability to produce clear assessment reports and support plans
Strong IT skills including Microsoft 365 and case management systems
Knowledge of safeguarding, confidentiality and data protection
Ability to drive with access to a car in order to carry out work around the London Borough of Bexley
Good understanding of the Care Act 2014, Mental Capacity Act 2005 and strengths-based practice (desirable)

Experience

Experience of working in adult social care, health or support services
Experience of supporting adults with learning disabilities, autistic people and/or other adults with care and support needs
Experience of record keeping, monitoring or audit processes
