Full job description

Care Coordinator / Field Supervisor (Domiciliary Care)

Location: Southgate (and covering nearby surrounding areas N14, N21. EN4)

The Employee shall undertake the duties of Care Coordinator / Field Care Supervisor and shall perform such duties as are necessary for the safe, effective, and efficient operation of the Employer's domiciliary care service. The Employee will support the Registered Manager in ensuring that high-quality, person-centred care is delivered in accordance with the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014, Care Quality Commission (CQC) Fundamental Standards, company policies, and all applicable legislation.

The Employee's duties shall include, but are not limited to:

Coordinating and managing staff rotas to ensure care visits are allocated efficiently and delivered safely, effectively, and in accordance with service users' assessed needs and preferences.
Planning and monitoring staff schedules, ensuring adequate staffing levels are maintained to meet business and service user requirements.
Monitoring staff attendance, punctuality, conduct, and performance, addressing concerns promptly and escalating issues where appropriate.
Conducting spot checks, competency assessments, supervision visits, observations, and field-based support to ensure staff deliver safe, effective, and person-centred care.
Carrying out service user assessments, care reviews, risk assessments, environmental assessments, and care plan updates to ensure care remains appropriate and reflects changing needs.
Supporting new care package implementations, introductions of care staff, and transitions of service users into the service.
Responding promptly to emergency situations, missed calls, staff absences, and changes in care requirements, arranging suitable cover as required.
Maintaining accurate, complete, and up-to-date records relating to service users, staff, incidents, visits, and operational activities using electronic care management systems and paper records where applicable.
Monitoring electronic call monitoring systems, investigating missed or late calls, and taking appropriate corrective action.
Processing and verifying staff timesheets, payroll information, mileage claims, and hours worked, assisting with the resolution of payroll queries.
Preparing, processing, and supporting invoicing and billing activities for local authority, NHS, private, and commissioned care packages, ensuring accuracy and timely submission.
Acting as a professional point of contact for service users, relatives, staff, healthcare professionals, local authorities, commissioners, and other external agencies.
Supporting recruitment activities, including interviews, onboarding, induction, Right to Work verification, DBS checks, references, and maintaining staff compliance records.
Monitoring mandatory training compliance and arranging refresher training to ensure staff remain competent and compliant.
Supporting medication competency assessments, practical observations, and ensuring staff follow medication policies and procedures.
Investigating accidents, incidents, safeguarding concerns, complaints, compliments, and near misses, ensuring appropriate reporting and follow-up actions are completed.
Promoting safeguarding, dignity, privacy, equality, diversity, inclusion, choice, and independence for all service users.
Participating in quality assurance activities, internal audits, mock inspections, action plans, and preparation for Care Quality Commission (CQC) inspections.
Liaising with healthcare professionals, GPs, district nurses, occupational therapists, social workers, commissioners, and multidisciplinary teams to ensure continuity of care.
Assisting with the development, implementation, and review of policies, procedures, and best practice guidance.
Supporting the Registered Manager in achieving and maintaining regulatory compliance and continuous service improvement.
Maintaining strict confidentiality and processing all personal information in accordance with the UK General Data Protection Regulation (UK GDPR), the Data Protection Act 2018, and company policies.
Promoting a positive, professional, and supportive working environment while acting as a role model for all care staff.
Participating in the on-call rota, where required, to provide operational support outside normal working hours.
Undertaking any mandatory training, supervision, meetings, or professional development required for the role.
Carrying out any other duties that are reasonable and commensurate with the responsibilities, seniority, and nature of the position, as directed by the Registered Manager or Employer.

The type of work may vary according to the operational needs of the Service. The Employee may be required to undertake additional duties consistent with the role of Care Coordinator / Field Care Supervisor, and appropriate instruction and support will be provided where necessary. The Employee is expected to work flexibly to meet the needs of the business while maintaining high standards of care, professionalism, and regulatory compliance at all times.
