Care Coordinator – Job Description

We are seeking a dedicated, organised, and compassionate Care Coordinator to join our team. The successful candidate will play a vital role in coordinating and managing client care plans, ensuring effective communication between clients, healthcare professionals, and support services.

This is a rewarding opportunity to contribute to the wellbeing of individuals while supporting the delivery of safe, effective, caring, responsive, and well-led services in line with Care Quality Commission (CQC) standards. The successful candidate will support the organisation in maintaining high-quality care delivery and achieving a CQC Good rating.

The ideal candidate must have a minimum of 5 years’ experience working within the UK health and social care sector, including experience within CQC-regulated services.

Key Responsibilities

Coordinate and schedule client appointments, care visits, treatments, and follow-up care.
Develop, implement, and monitor person-centred care plans to ensure they meet the individual needs of clients.
Maintain accurate, up-to-date care records and documentation in line with organisational policies and regulatory requirements.
Act as a key point of contact between clients, families, healthcare professionals, and support staff to ensure clear and effective communication.
Support the day-to-day coordination of care services to ensure continuity, safety, and quality of care.
Monitor client progress and update care plans where required.
Conduct occasional site visits, client reviews, and staff support visits where necessary.
Assist with administrative duties including data entry, documentation, filing, and correspondence.
Coordinate resources and support services tailored to individual client needs.
Ensure compliance with Care Quality Commission (CQC) regulations, safeguarding policies, and confidentiality standards.
Support internal audits, quality monitoring, and service improvement initiatives to help the organisation achieve and maintain a CQC Good rating.

Qualifications and Experience

Minimum of 5 years’ experience working within the UK health and social care sector.
Experience working within a CQC-regulated service and strong understanding of CQC Fundamental Standards.
Previous care coordination, administrative, or office experience within a care setting is highly desirable.
Knowledge of care planning, safeguarding procedures, and quality assurance in health and social care.
Excellent organisational and time-management skills.
Strong written and verbal communication skills.
Compassionate, professional, and committed to person-centred care.
Ability to work independently and as part of a multidisciplinary team.
Proficiency in standard office software programmes (e.g., MS Office).
Must hold a valid UK driving licence and have access to a vehicle, as travel may be required to visit clients and services.
Strong attention to detail and high standards of record keeping.
