Overview

We are seeking an organised, compassionate and proactive Care Coordinator to join our team in delivering exceptional, person-centred care within our Care Organisation. The ideal candidate will have experience coordinating care services within Adult Social Care and will play a key role in ensuring the smooth planning, scheduling and delivery of domiciliary care and supported living services.

Working closely with the Deputy Manager and Registered Manager, the successful candidate will coordinate care schedules, support service users and care staff, maintain accurate records and ensure care is delivered safely, effectively and in line with individual care plans. This role requires excellent organisational, communication and problem-solving skills, alongside a commitment to delivering high-quality care that reflects our values of Caring, Integrity, One Team and Responsibility.

Responsibilities

Coordinate and monitor the delivery of high-quality, person-centred care services.
Manage daily care schedules, staff rotas and service requirements to ensure continuity and reliability of care.
Allocate care staff effectively, taking into account individual needs, staff skills, availability and continuity of care.
Respond promptly to changes in care needs, emergencies and operational issues, ensuring appropriate action is taken.
Support the implementation of new care packages, including coordinating assessments, introductions and service commencement.
Monitor missed visits, late calls and service concerns, taking appropriate action to resolve issues.
Act as a key point of contact for service users, families, care staff and external professionals, promoting positive and effective communication.
Liaise with healthcare professionals, commissioners, social workers and other agencies to ensure coordinated care delivery.
Support the development, review and maintenance of person-centred care plans, risk assessments and care documentation.
Maintain accurate electronic and paper-based records, ensuring compliance with CQC standards, organisational policies and GDPR requirements.
Assist with quality assurance activities, audits, service monitoring and continuous improvement initiatives.
Provide day-to-day guidance and support to care staff, assisting with workforce coordination and rota management.
Monitor staff compliance with mandatory training, documentation and organisational procedures.
Prepare reports and provide operational updates to the Deputy Manager and Registered Manager as required.

Skills

Proven experience working within Adult Social Care, healthcare or a similar regulated environment.
Experience coordinating care services, staff rotas or workforce scheduling.
Good understanding of person-centred care, safeguarding principles and CQC regulations.
Excellent organisational and time-management skills, with the ability to prioritise competing demands.
Strong communication and interpersonal skills, with the ability to build positive relationships with service users, families, colleagues and external professionals.
Ability to problem solve, make informed decisions and respond effectively to changing service needs.
Experience maintaining accurate records and handling confidential information in line with GDPR requirements.
Proficient in Microsoft Office and digital care management systems.
Ability to work independently while contributing positively as part of a collaborative team.
Compassionate, reliable and committed to delivering safe, high-quality care that promotes independence, dignity and wellbeing.
A Level 3 qualification in Health and Social Care or equivalent is desirable, together with knowledge of care planning, quality assurance and compliance within regulated care services.

Pay: £13.75-£15.00 per hour
