Location: Abbeycare Scotland – Erskine Mains House PA8 7ED

Reports to: Operations Director

Salary: £37.500.00 - £40.000.00

Hours: Full-Time 40hrs per week

Contract: Permanent

About Abbeycare Scotland

Abbeycare Scotland is a residential abstinence-based treatment service providing supervised detoxification and rehabilitation for individuals recovering from substance use disorders. Our trauma-informed therapeutic programme integrates Cognitive Behavioural Therapy (CBT) and the 12-Step recovery model to support clients in achieving and maintaining long-term abstinence. Our aftercare services are a vital component of our service, providing ongoing support to graduates as they transition from residential treatment to independent recovery in the community.

Job Purpose

The Aftercare Outreach Coordinator is responsible for leading, planning, delivering, and evaluating the aftercare support programme for former clients. The post-holder will provide one-to-one and group support, coordinate follow-up contact, and ensure effective data management and communication between the clinic, clients, and community partners. The role is central to sustaining long-term recovery outcomes and preventing relapse through continued engagement, encouragement, and support.

Key Responsibilities

· Client Support and Engagement

· Deliver structured aftercare follow-up calls, providing emotional and practical support post-discharge.

· Offer one-to-one recovery check-ins and guidance tailored to individual client needs.

· Facilitate aftercare and peer-support groups using trauma-informed, CBT-based, and 12-Step approaches.

· Support clients to maintain recovery connections, develop relapse prevention strategies, and access community recovery networks.

· Work collaboratively with recovery practitioners and therapists to ensure continuity of care post-residency.

Programme Coordination and Administration

· Develop and maintain an effective aftercare tracking system, ensuring accurate and timely data entry.

· Record all client contacts, group attendance, and outcome measures in line with service protocols and GDPR requirements.

· Monitor client engagement and follow up with individuals who are disengaged or are at risk of relapse.

· Produce regular reports on aftercare outcomes, trends, and client feedback for service evaluation.

Community and Partnership Working

· Build relationships with local recovery networks, mutual aid groups, and partner agencies.

· Signpost clients to community-based services such as addiction recovery services, mental health, housing, education, training, and employment support.

· Represent Abbeycare at relevant community recovery events and meetings.

· Develop Abbeycare outreach services in the community.

Teamwork and Professional Practice

· Work collaboratively with the residential team to ensure smooth transition planning for clients leaving treatment.

· Attend team meetings, case reviews (where appropriate), and reflective practice sessions.

· Contribute to service development by identifying opportunities to enhance aftercare provision and services.

· Uphold professional boundaries, confidentiality, and ethical standards at all times.

Health, Safety, and Compliance

· Adhere to all safeguarding, confidentiality, and data protection policies.

· Follow risk management procedures and report any concerns in accordance with service protocols.

· Promote a safe, inclusive, and supportive environment for all clients.

Person Specification

Essential

· Experience of working in substance use, mental health, or social care settings.

· Ability to lead, motivate and manage the outreach department.

· Understanding trauma-informed care and recovery-oriented practice.

· Strong communication and interpersonal skills with the ability to build trust and motivate clients and team members.

· Good organisational skills with attention to detail in record keeping and data management.

· Confident in facilitating group sessions and providing one-to-one support.

· Ability to work independently and as part of a multidisciplinary team.

· Ability to network with an ability to identify opportunities for service development within the community.

· Basic IT skills including Microsoft Office and database systems.

· Flexibility to work occasional evenings or weekends.

Desirable

· SVQ Level 4 (Health and Social Care) or equivalent qualification, or willingness to work towards it.

· Experience in a leadership role.

· Knowledge or lived experience of the 12-Step model and/or peer recovery support.

· Training in CBT, Motivational Interviewing, or group facilitation.

· Experience delivering outreach or aftercare services.

· Registration with or eligibility for registration with the Scottish Social Services Council (SSSC).
