Full job description

Positive Steps is a Non-Denominational Christian charity; working in Dundee since 1989 we support some of the most vulnerable people living in our community and are providing a similar service in Perth. We have a reputation for recognising local need and delivering innovative solutions. Through our community based support we engage with individuals facing a range of challenges including substance use, homelessness, mental health problems, and offending behaviour. We strive to build trusting and consistent relationships in which individuals are supported to recognise and address their difficulties and make lasting positive changes to their lives.

Our Project Workers support people with a range of challenges and problems living in their communities. We work with people to agree and provide the support they need and want. Everyone we work with is treated as an individual.

As an Outreach Project Worker you will interact with a caseload of Service Users who have a range of complex needs. A practical knowledge of homelessness, harm reduction, dependency issues, benefits and the criminal justice system are crucial to the role. SVQ II/III Health and Social Care (Adult), or equivalent, is desirable although this is not essential as a comprehensive induction and a training package, which includes the necessary SVQs, is provided.

The post is subject to satisfactory references (one current or most recent employer and one character) and membership of the PVG (Adult) Scheme. Daily use of a roadworthy, appropriately insured car is required.

Key Functions:

Provide advice, advocacy, assistance and support to vulnerable adults.
Liaise with and carry out needs assessment with service users.
Liaise and work with members of Positive Steps staff, staff from relevant external agencies, key professionals and service users.

Main Duties:

Provide advice, advocacy, assistance, and support to a caseload of service users within agreed individual action plans.
Support service users to identify and address issues relating to their crisis triggers and signpost and support individuals to source and engage with correct and required support.
Assess and monitor individual service user needs on a regular basis and report any changes to senior staff and external agencies as appropriate.
Review and reassess risk assessments for service users and staff (to include Lone Working)
Ensure online/computer-based reporting systems are kept up to date.
Review and reassess service user needs with them, external agencies, key persons and senior staff in agreed timescales.
Develop person centred action plans in conjunction with the service user.
Enable the service user to make informed choices and take informed risks.
Attend and host review or network meetings when required.
Ensure services are delivered to the agreed professional practice standard.
Maintain clear, accurate and up-to-date service user records.
Attend regular team meetings to actively discuss and implement changes to policies, procedures, or practices.
Develop one’s own knowledge and practice. Attend training courses and undertake courses of study as agreed with senior staff.
Maintain confidentiality of service users and Positive Steps as laid down in the Staff Handbook.
Any other duties, to the equivalent level, which are necessary to the purpose of the post.

Special Terms
Must have daily use of a legal, roadworthy vehicle with appropriate insurance (maximum HMRC mileage allowance paid).
