Community Support Worker – Complex Physical Health Support for Children & Young People in Hampshire.

We are looking for compassionate, experienced Support Practitioners to join our growing team in Hampshire.

This role offers a mix of flexible shorter shifts (2–6 hours) and longer wraparound support (days and waking nights 8–12 hours), supporting children and young people with high support needs in the community.

Who We Are

Care Bridge Specialists support individuals with autism, learning disabilities, mental health challenges, behaviours of concern, challenging social circumstances and complex needs, who are often considered too complex for standard services.

We are CQC registered to provide specialist, person-centred support to help people remain safely in the community, avoid hospital admission, and live meaningful, fulfilling lives.

Competitive pay: £16–£18 per hour (depending on experience)

Zero-hours contract during initial 6-month probation

About the Role

You will be part of a Practitioner team supporting children and young people with complex, high-acuity needs, including autism, learning disabilities, mental health comorbidities and complex physical health needs.

We are particularly looking for Community Support Workers with experience supporting children and young people requiring specialist clinical interventions within their own homes and community settings.

You will:

Deliver person-centred support in line with care plans and risk assessments
Use Positive Behaviour Support (PBS) and de-escalation approaches
Support with daily living, routines, community access and independence
Build trusting, therapeutic relationships with the individuals you support
Work collaboratively with families, professionals and clinical teams
Safely support delegated healthcare tasks following training, competency assessments and clinical guidance
Monitor health needs and escalate concerns appropriately

We Are Looking for Staff with Experience in - Complex Physical Health Support

Tracheostomy care
PEG feeding
Nasogastric (NG) tube feeding
Suctioning
Ventilation support
Oxygen therapy
Colostomy care
Ileostomy care
Urostomy care
Gastrostomy and enteral feeding
Jejunostomy (PEJ/JEJ) feeding
Oral and nasopharyngeal suction
Non-invasive ventilation (BiPAP/CPAP)
Cough Assist devices
Nebuliser administration
Epilepsy management and rescue medication
Diabetes management and blood glucose monitoring
Catheter care (urethral and suprapubic)
Bowel management programmes
Continence care
Pressure area care and prevention
Moving and handling using specialist equipment
Hoisting and complex manual handling
Spinal injury support
Neurological conditions
Medication administration (including complex medication regimes)
Health observations (including NEWS2 where applicable)
Emergency response for clinically complex children and young people

What We're Looking For - Essential Criteria

Business use UK Driving Licence and access to a vehicle
Motor Business Insurance (evidence required)
Current MOT certificate
Certificates of evidence of training in specialisms as listed above
Up to 5+ years' experience in health or social care (Band 4 equivalent)
Experience supporting children or young people with complex needs
Strong understanding of autism, learning disabilities, mental health and/or complex physical healthcare needs
Strong communication and record-keeping skills, including clear written notes, handovers and incident reporting, with confidence using electronic care systems (e.g. Nourish) and contributing to MDT and professional meetings
NVQ Level 2 (minimum) or Care Certificate
Full UK driving licence
Willingness to complete specialist training
Experience supporting behaviours of concern and crisis presentations
Training in PMVA or CPI Physical Safety Interventions and Positive Behaviour Support
Experience across ATUs, hospitals, secure residential services and community settings

Desirable

Competency in one or more complex clinical skills listed above
Experience supporting children with medical complexity, life-limiting conditions or long-term health conditions in community settings
Experience working alongside Community Children's Nurses, Continuing Care Teams or Complex Care providers
