Full job description

New Era Residence is a growing, values-led care provider supporting young people and adults aged 17 to 25 with mental health needs, learning disabilities and/or autism in residential settings. Our services are registered with the Care Quality Commission and are built around person-centred, strengths-based practice.

We are committed to enabling people to live meaningful, independent, and fulfilling lives within their local communities. Our approach is proactive, therapeutic, and preventative, with a strong focus on positive behaviour support, risk reduction, and promoting well-being.

We use Nourish as our electronic care planning and recording system to ensure accurate, timely, and outcome-focused documentation that meets regulatory and best practice standards.

Role Purpose

The Team Leader plays a key role in leading and delivering safe, effective, responsive, and well-led support in line with CQC regulations. You will support residents with mental health conditions, learning disabilities and/or autism to develop independence, life skills, emotional wellbeing, and positive outcomes.

You will work proactively to:

Reduce risks and promote positive behaviour
Support residents to achieve personalised outcomes identified in their care and support plans
Record high-quality, accurate care notes using Nourish

Key Responsibilities

Effectively lead shifts, direct and support colleagues.
Provide day-to-day support in line with individual care plans, risk assessments, and Positive Behaviour Support (PBS) plans
Promote independence, choice, control, and inclusion in all aspects of daily living
Support residents with personal care where required, maintaining dignity and respect at all times
Encourage and support residents to access education, employment, training, leisure, and community activities

Proactive & Positive Behaviour Support

Use proactive strategies to prevent distress, reduce risk, and promote emotional regulation
Respond calmly and professionally to incidents, behaviours of concern, or crisis situations
Follow agreed de-escalation techniques and restrictive practice guidance (where applicable)
Contribute to behaviour monitoring and reviews to support continuous improvement

Safeguarding & Risk Management

Recognise, respond to, and report safeguarding concerns in line with local authority and organisational procedures
Promote a culture of openness, whistleblowing, and professional curiosity
Carry out health and safety checks, environmental audits, and risk assessments
Support safe lone working practices

Recording & Compliance

Maintain accurate, timely, and outcome-focused records using Nourish, including daily notes, incident reports, and health updates
Contribute to reviews of care plans, risk assessments, and outcome measures
Understand and apply the Mental Capacity Act (2005) and best interest decision-making

Teamwork & Service Development

Build positive and professional relationships with residents, colleagues, families, and external professionals
Act as a role model for good practice and professional conduct
Participate in shift handovers and contribute to effective communication within the team
Support managers with audits, service reviews, and quality assurance activities
Where required, take on shift-leading responsibilities

Essential Criteria

Aged 18 years +
Driving Licence (preferred)
6 months experience supporting adults with learning disabilities, autism, and/or complex needs (preferred)

Understanding of:

Safeguarding adults & children
Whistleblowing procedures
Positive Behaviour Support and managing behaviours of concern
Professional boundaries
Ability to remain calm and responsive during challenging situations
Strong verbal and written communication skills
Willingness to work flexibly, including weekends, nights, and bank holidays

Desirable Criteria

Level 3 Diploma in Health & Social Care (or equivalent)
Care Certificate
Experience working in CQC-registered residential services
Understanding of Outcome Stars, life skills frameworks, or similar outcome tools
Experience working with children and young people
Experience in working in transitional services

Training & Development

New Harmony Care Group is committed to workforce development. You will:

Complete a comprehensive induction programme
Undertake mandatory training including safeguarding, PBS, MCA, health & safety, and autism awareness
Be supported to enrol and work towards level 3 & 4
Participate in supervision, appraisal, and continuous professional development
