Full job description

Job Overview

We are looking for a compassionate, organised, and proactive Care Coordinator who holds a full UK driving licence and is able to travel throughout Southampton and the surrounding areas. The successful candidate will be responsible for coordinating care services, supporting care staff, conducting client visits, and ensuring high-quality, person-centred care is delivered to clients in their own homes.

Key Responsibilities

Coordinate and schedule care staff to ensure efficient service delivery.
Conduct care assessments, reviews, and risk assessments for new and existing clients.
Travel to clients' homes across Southampton to monitor the quality of care and provide support where needed.
Maintain accurate care plans, daily records, and documentation.
Liaise with clients, families, healthcare professionals, and local authorities to ensure continuity of care.
Respond promptly to emergencies, staff absences, and changes in care requirements.
Support the recruitment, induction, supervision, and ongoing development of care staff.
Carry out spot checks, competency assessments, and quality assurance visits.
Ensure compliance with CQC standards, safeguarding procedures, health and safety, and company policies.
Participate in the on-call rota as required.

Essential Requirements

Full UK driving licence with access to a reliable vehicle.
Previous experience in domiciliary care, health and social care, or care coordination.
Excellent organisational and time management skills.
Strong communication and interpersonal skills.
Good IT skills and experience using care management systems.
Ability to work independently and manage multiple priorities.
Flexible approach to working hours, including occasional evenings, weekends, and on-call duties.

Desirable Qualifications

NVQ/QCF Level 3 in Health and Social Care (Level 5 desirable).
Knowledge of CQC regulations and person-centred care practices.
Experience in rota planning and staff coordination.
