Job Overview

We are looking for a Practice Nurse to work between 30 - 37 hours per week, pay will be dependant on experience.

We are seeking a resilient, flexible, and adaptable Practice Nurse to join our highly skilled clinical team. This position is ideally suited for a qualified nurse with a minimum of two years post-registration experience.

Whether you have an established background in primary care, community nursing, or an urgent treatment centre, or you are currently managing long-term conditions in secondary care and looking to transition into General Practice, we want to hear from you. You will be a confident practitioner, comfortable working autonomously without direct supervision, and capable of making and reporting independent clinical decisions.

Ideal candidates will have experience in a General Practice setting.

We are a dedicated Training Practice, supporting specialist registrars, foundation doctors, and medical and nursing students.

With a registered list size of over 14,000 patients, we manage a diverse range of complex, stimulating, and challenging presentations. This high demand is coordinated by an inspirational and motivated team of professionals.

Duties

As a core member of our clinical team, your primary responsibilities will include:

Patient Care & Chronic Disease Management

Provide routine nursing care in accordance with clinical evidence, local, and national guidance.
Implement and evaluate individual treatment plans for chronic disease patients, maintaining accurate chronic disease registers.
Identify, manage, and support patients at risk of developing long-term conditions to prevent adverse health effects.
Run chronic disease clinics, referring patients to secondary or specialist care when required.
Support patients with the safe use of prescribed or over-the-counter medicines within your scope of practice.
Provide comprehensive travel medicine services.
Request and process pathology services as necessary.
Prioritise health issues and intervene appropriately, supporting the team during clinical emergencies.
Recognise, assess, and refer patients presenting with mental health needs.
Liaise with external agencies to ensure vulnerable patients are fully supported.
Support the clinical team with all safeguarding matters in accordance with local and national policies.
Maintain exceptional and accurate clinical records.

Experience

Proven experience working as a Practice Nurse or in a similar healthcare setting.
Strong knowledge of anatomy, physiology, and common medical conditions.
Excellent patient care skills with the ability to communicate effectively and empathetically.
Familiarity with clinical procedures, infection control standards, and healthcare regulations.
Ability to work efficiently both independently and as part of a team.
Prior experience with health promotion initiatives or chronic disease management is desirable but not essential.

Pay: £30,784.00-£38,480.00 per year
