Full job description

Care Co‑ordinator – All About You Care Services

Poulton‑le‑Fylde | £26,247 per year | Full‑time (37.5 hrs) or 30 hrs option | Office‑based

All About You Care Services are looking for an organised, compassionate and confident Care Co‑ordinator to join our growing community care team. This is a key role in ensuring safe, high‑quality, person‑centred care for the people we support.

Key Responsibilities

Scheduling and coordinating care visits to ensure continuity and safe staffing
Being the first point of contact for carers, service users and families
Supporting care plans, risk assessments and reviews
Monitoring visit compliance, punctuality and quality of care
Responding to changes, emergencies and unplanned events calmly and professionally
Working closely with the Registered Manager to maintain CQC standards
Supporting recruitment, inductions and staff supervision
Participating in the on‑call rota (paid in addition to salary)

About You

Experience in domiciliary care coordination or a senior care role
Strong understanding of safeguarding, duty of care and person‑centred practice
Excellent communication and problem‑solving skills
Confident using digital scheduling and care management systems
Able to work in a fast‑paced environment with competing priorities
Willing to support in the community when required

Benefits

Full‑time 37.5 hours per week (office based in Poulton‑le‑Fylde)
Salary: £26,247 per annum
Option for a 30‑hour contract for suitable candidates
Paid on‑call
Company pension scheme
