Full job description

TP Care at Homes Ltd is recruiting dedicated and resilient Health Care Asstsistant to support a young person living in the Wareham area within their home and local community.

This is a complex care package, behaviours that challenge, and emotional turmoil possible. The successful candidates will need to provide calm, consistent, trauma-informed and person-centred support, while maintaining safety, dignity and structure.

The package is delivered on a 24/7 basis through planned shifts, so flexibility is essential.

About the role
You will work as part of a team to support the young person with their daily routine, safety, emotional wellbeing and community access. The role requires a high level of vigilance, professional curiosity, accurate reporting and the ability to respond appropriately in dynamic situations. You will be expected to build trust, maintain professional boundaries, follow support plans and risk assessments, and work closely with family members and professionals where required.

Duties may include providing support with daily living, community access, emotional regulation, positive behaviour support, safeguarding, observation, documentation, missing-person protocols, and promoting the individual’s independence and quality of life.

What we are looking for
We are seeking applicants who are compassionate, dependable and emotionally resilient, with a genuine commitment to person-centred care. Experience supporting people with behaviours that challenge, mental health needs, autism, learning disabilities or complex community-based packages would be advantageous.

Applicants should be able to:

Work effectively as part of a team
Follow care plans, behaviour support plans, risk assessments and safeguarding procedures
Remain calm under pressure and respond appropriately to incidents
Communicate clearly and professionally
Maintain accurate records and handovers
Support safe participation in the community
Work flexible shifts across a 24/7 rota

A full UK driving licence may be desirable depending on the package requirements.

Essential requirements
Successful applicants will be expected to meet safer recruitment and compliance requirements appropriate to care work. This may include:

Enhanced DBS check, including the appropriate workforce and barred list checks where the role is eligible
Satisfactory references
Proof of right to work in the UK
Suitability to work with children and/or adults at risk, depending on the final package arrangement and legal eligibility for checks
Willingness to complete mandatory training, including safeguarding, moving and handling, medication awareness, health and safety, infection prevention and control, and positive behaviour support
