Job purpose

To provide safe, compassionate and effective nursing care to patients across the practice population as part of the multidisciplinary primary care team. The role is designed to support a newly qualified nurse, a nurse in the first one to two years of practice, or a registered Nursing Associate who is keen to develop a career in general practice.

The postholder will deliver a range of treatment-room and long-term-condition support activities, initially working within existing competence and progressing through a structured programme of training, supervision and competency assessment.

Healthcare assistants and administrative colleagues

Main duties and responsibilities

Clinical care

Wound care and dressings: assess wounds, plan and deliver appropriate care, document progress and escalate concerns. Undertake leg-ulcer care and compression therapy once the required assessment skills, training and competencies have been completed and in accordance with the agreed care plan.

Immunisations and vaccinations: provide adult and childhood immunisations when trained, assessed as competent and authorised through the appropriate legal mechanism and local policy. Maintain vaccine safety, cold-chain standards and emergency preparedness.

Cervical screening: undertake cervical sample taking after completing the recognised NHS Cervical Screening Programme training pathway, local registration and competency requirements. Where the postholder is a Nursing Associate, this will be a delegated activity with the required professional support and supervision arrangements in place.

Blood sampling: occasionally undertake venepuncture, correctly label and prepare specimens, and act appropriately where a patient becomes unwell or where a specimen cannot be obtained.

ECGs and observations: record high-quality 12-lead ECGs and physiological observations, recognise when findings or symptoms require urgent review, and seek timely clinical support.

Health promotion: use each contact to support prevention, self-care and healthy lifestyle choices, adapting communication to the patient's needs.

Patient assessment: work within competence to identify deterioration, safeguarding concerns or other clinical risk and promptly escalate to an appropriate registered clinician.

Safe and effective practice

Work within the NMC Code, professional standards, current competence and the practice's clinical governance arrangements.

Obtain valid consent, maintain privacy and dignity, and support reasonable adjustments and equitable access.

Keep clear, accurate and contemporaneous electronic patient records using the practice clinical system.

Follow infection prevention and control procedures, including safe use and disposal of sharps and clinical waste.

Maintain emergency awareness, including basic life support and anaphylaxis training appropriate to the role.

Manage medicines and vaccines safely and only within the postholder's legal authority, professional scope and practice authorisation.

Report incidents, near misses, equipment problems and clinical concerns promptly and contribute to learning and improvement.

Maintain confidentiality and comply with information governance, safeguarding, health and safety and chaperoning requirements.

Teamwork and communication

Build positive and respectful relationships with patients, carers and colleagues.

Work collaboratively with the nursing and wider multidisciplinary team, sharing relevant information and seeking advice when needed.

Contribute to practice meetings, nursing meetings, audits and quality-improvement activity.

Support effective stock control, safe storage of clinical supplies and readiness of treatment rooms and equipment.

Participate in appropriate seasonal and practice vaccination programmes as agreed within the working pattern.

Learning and development

Participate fully in induction, preceptorship or an agreed development programme.

Complete supervised learning and competency assessment for the duties included in the individual job plan.

Maintain a development portfolio and meet NMC revalidation requirements.

Engage in clinical supervision, reflective practice, appraisal and mandatory training.

Develop additional primary care skills over time in line with service need, personal interest and safe scope of practice.

General responsibilities

Work flexibly within the agreed four-day pattern to meet patient and service needs.

Undertake other reasonable duties appropriate to the grade, registration, competence and purpose of the post.

Promote the practice's values and contribute to a supportive, inclusive and learning-focused workplace.
