Full job description

About the Role

We are looking for a dedicated, organised and proactive Care Manager to join our growing team.
You will play a key role in the day-to-day management and delivery of high-quality care services.
You will oversee care workers and senior care workers, ensuring care schedules remain safe, effective and well-coordinated.
You will provide leadership, guidance and support to staff while ensuring service users receive person-centred, safe and high-quality care.
The successful candidate will be confident working independently, managing competing priorities and building positive relationships with service users, families, staff and external professionals.

Key Responsibilities

Oversee the day-to-day operation of the care service and ensure effective service delivery.
Manage and monitor staff performance, development, attendance and conduct.
Plan, review and optimise staff rotas and care schedules to ensure appropriate staffing levels and continuity of care.
Liaise with health and social care professionals to process referrals promptly and coordinate safe interventions.
Conduct initial assessments and service user visits to identify individual care and support needs.
Develop detailed, person-centred care plans and ensure these are regularly reviewed and updated.
Review care packages on a regular basis or when service user needs, abilities or circumstances change, ensuring visit times and care provision remain appropriate.
Complete and regularly review risk assessments, including moving and handling assessments.
Ensure identified risks are appropriately managed and information is shared with relevant professionals, staff and other appropriate parties.
Supervise and support care workers through regular supervisions, appraisals and spot checks.
Monitor staff performance and take appropriate action where required.
Support the induction and development of new staff, including Care Certificate training and competency assessments.
Monitor the quality of care provided and identify opportunities for continuous improvement.
Maintain accurate and up-to-date records using relevant care management systems and Microsoft Office applications.
Provide regular feedback and operational updates to the Registered Manager.
Respond appropriately to complaints, accidents, incidents and safeguarding concerns, ensuring they are investigated and managed in accordance with company policies and procedures.
Promote and maintain the health, safety, dignity, independence and wellbeing of all service users.
Work collaboratively with families, professionals, local communities and other stakeholders to achieve positive outcomes for service users.
Participate in relevant local and national development programmes and contribute to the continuous improvement of the service.
Ensure all work is carried out in accordance with relevant legislation, regulations, policies and best practice guidance.
Participate in on-call to ensure service continuity (additional pay outside of salary).

About You

Level 3 qualification in Health and Social Care or equivalent experience.
Previous experience in a care management, senior care or supervisory role.
Strong understanding of person-centred care and safeguarding.
Experience supporting, supervising and developing care staff.
Experience completing or contributing to care plans, risk assessments and reviews.
Excellent organisational and time-management skills, with the ability to prioritise competing demands.
Strong problem-solving skills and the ability to respond proactively to changing circumstances.
Excellent communication and interpersonal skills, with the ability to deal sensitively and professionally with service users, families, staff and external professionals.
Ability to work effectively with stakeholders at all levels, using tact, diplomacy and professionalism.
Confident using Microsoft Office and care management software.
High level of accuracy when recording, updating and maintaining confidential information.
Good understanding of GDPR, confidentiality and information governance, including the principles relevant to the Freedom of Information Act.
Ability to work independently, use initiative and meet deadlines with minimal supervision.
A commitment to quality improvement and delivering excellent outcomes for service users.
