Yellowstone Healthcare Ltd is seeking compassionate, dedicated and resilient Support Workers to join our Adult Supported Accommodation services in Seven Kings/Ilford and Basildon.

This is an exciting opportunity to work with adults with learning disabilities, autism, mental health conditions, physical disabilities and other complex support needs, providing high-quality, person-centred care in accordance with CQC Fundamental Standards.

We are looking for individuals who are committed to promoting independence, dignity, choice and positive outcomes while delivering safe and effective support.

About the Role

As a Support Worker, you will:

Deliver high-quality, person-centred care and support tailored to each individual's assessed needs.
Support adults with complex needs, learning disabilities, autism, mental health conditions and physical disabilities.
Provide personal care, including assistance with washing, dressing, grooming, continence care and maintaining dignity and privacy.
Administer, prompt and record medication in accordance with Medication Administration Records (MAR), company policies and competency requirements.
Support individuals with mobility, moving and handling, nutrition and hydration.
Promote independence by encouraging people to develop daily living and life skills.
Support individuals to prepare meals, undertake household tasks and maintain a safe, clean living environment.
Support individuals to access healthcare appointments, education, employment opportunities, volunteering and community activities.
Develop positive therapeutic relationships based on respect, dignity and trust.
Follow individual Care Plans, Risk Assessments, Positive Behaviour Support (PBS) Plans and Behaviour Support Plans.
Support individuals who may present with behaviours that challenge, using de-escalation techniques, Positive Behaviour Support and trauma-informed approaches.
Recognise and report changes in physical health, mental health and behaviour promptly.
Complete accurate daily records, care notes, MAR charts, incident reports and other required documentation.
Work collaboratively with families, advocates, social workers, community nurses, psychologists, psychiatrists, occupational therapists and other multidisciplinary professionals.
Promote safeguarding, human rights, equality, diversity and inclusion in all aspects of support.
Participate in key working responsibilities where allocated.
Support individuals to manage finances, budgeting and shopping where appropriate.

Working as Part of the Team

You will also:

Work collaboratively with colleagues to deliver outstanding person-centred care.
Attend supervision, team meetings and training sessions.
Participate in reflective practice and continuous professional development.
Follow all safeguarding, infection prevention and control, medication, health and safety and company policies.
Report incidents, accidents, safeguarding concerns and near misses promptly.
Promote a positive, recovery-focused culture.
Participate in quality assurance and continuous improvement activities.
Be flexible to work across services where required.

What We're Looking For

Essential

Minimum 6–12 months' experience working within Adult Health and Social Care.
Experience supporting adults with complex needs, learning disabilities, autism, mental health conditions and/or physical disabilities.
Experience providing personal care.
Experience administering or prompting medication (or willingness to complete competency training).
Experience supporting individuals who present with challenging behaviours, emotional distress or behaviours of concern.
Good written and verbal communication skills.
Ability to work effectively as part of a multidisciplinary team.
A caring, patient, compassionate and resilient approach.
Commitment to safeguarding vulnerable adults.
Ability to work shifts, weekends and Bank Holidays.
Right to work in the UK.

Desirable

Level 2 or Level 3 Diploma in Adult Care, Health & Social Care or equivalent qualification.
Training in Positive Behaviour Support (PBS).
Medication Administration training.
Moving and Handling training.
Autism awareness training.
Mental Health Awareness training.
Full UK Driving Licence.

Working Pattern

Full-time and Part-time opportunities available.
12-hour shifts.
Days, waking nights, weekends and Bank Holidays on a rota basis.
