Full job description

JOB PURPOSE

TogetherWORKS is an open access health and wellbeing hub that supports people to improve their wellbeing, reduce social isolation and connect with their community. We provide a range of creative, educational and wellbeing activities, alongside signposting. Providing support to access services provided by health, social care and third sector partners.

The Wellbeing Facilitator will provide early intervention and preventative support to people experiencing declining wellbeing, social isolation, learning disabilities, neurodivergence, neurodevelopmental conditions and emotional wellbeing challenges.

Working as part of the Integrated Community Care System (ICCS), the postholder will support individuals to access community activities, volunteering opportunities and appropriate services before issues escalate into crisis. The role will work closely with GAVO Wellbeing Link Workers, Aneurin Bevan University Health Board services, local authority teams and voluntary sector organisations to ensure people receive timely and appropriate support.

The Wellbeing Facilitator will help people build confidence, increase community participation and maintain independence through one-to-one support, group activities and effective signposting. Accurate record keeping, monitoring and evaluation are integral parts of the role.

DUTIES

Support people with learning disabilities, neurodivergence and emotional wellbeing needs to access community opportunities.
Provide one-to-one wellbeing support and check-ins for individuals.
Develop bespoke volunteering opportunities and assist with induction and support continued engagement.
Develop community groups and activities based upon unmet needs in the community
Support people to access existing TogetherWORKS activities, community groups and wellbeing opportunities.
Identify emerging needs and provide appropriate information, advice, signposting
Support individuals to access volunteering opportunities and engage with community provision at TogetherWORKS
Work closely with Wellbeing Link Workers, health professionals, local authority teams and community organisations.
Develop positive relationships with participants, carers, families and partner organisations.
Maintain accurate records, case notes and monitoring information.
Collect participant feedback, outcomes and case studies to demonstrate project impact.
Produce reports and information required by funders, partners and stakeholders.
Promote project activities through appropriate publicity and engagement activities.
Attend meetings with funders, partners and stakeholders as required.
Support the smooth day-to-day running of TogetherWORKS activities and events.
Support and work alongside volunteers where appropriate.
Ensure safeguarding, health and safety, confidentiality and data protection procedures are followed at all times.
Work within TogetherWORKS policies, procedures and governance framework.
Undertake any other duties that may reasonably be required by the organisation.

Pay: £29,419.00 per year
