Job Overview

We are seeking a dedicated and compassionate Care Coordinator to join our team. The successful candidate will play a vital role in organising and managing care plans, ensuring clients receive the highest quality support tailored to their individual needs. This position offers an opportunity to make a meaningful difference in people's lives while working within a supportive and professional environment. The role is paid and suitable for individuals passionate about providing excellent care and support services.

Duties

Coordinate and develop personalised care plans in collaboration with clients, families, and healthcare professionals.
Act as a primary point of contact for clients, offering guidance and support throughout their care journey.
Organise appointments, liaise with service providers, and ensure timely delivery of care services.
Maintain accurate records of client interactions, care plans, and progress reports using IT systems.
Provide information and advice to clients regarding available services and resources.
Monitor client wellbeing, addressing concerns promptly and escalating issues when necessary.
Support training sessions or educational activities related to care provision when required.

Skills & Experience

Previous experience as a Care Coordinator, Scheduler, or Senior Care Worker within a domiciliary care setting.
Strong communication and interpersonal skills, with the ability to build positive relationships with clients, families, care staff, and healthcare professionals.
Experience creating and managing care staff rotas using electronic rostering systems.
Good understanding of CQC regulations and person-centred care.
Strong organisational skills with the ability to manage multiple cases efficiently.
Proficient IT skills, including experience with data entry, scheduling software, and electronic health records.
Experience or background in teaching or training is advantageous, enabling effective support and education for clients or team members.
Empathy, patience, and a genuine desire to improve the quality of life for others are essential qualities for success in this role.
