CARE COORDINATOR

Contract Type: Full-Time hours

Salary: £32,000.00 Per Year

Specific Hours: Full-time (37.5 hours per week, flexibility required including evenings, weekends and participation in the on-call rota)

Requirements:

Highly motivated, ambitious, and passionate about delivering high‑quality care services
Strong attention to detail and accuracy when managing schedules and reports
Ability to work collaboratively with Care Managers, care workers, and other team members
Previous experience as a Care Coordinator in Health & Social Care (minimum of 2 years)
Experience with electronic care planning/rostering systems
Proven ability to manage and maintain a high‑quality, dynamic care service
Excellent people management and communication skills
Strong leadership and teamwork abilities
NVQ Level 3 in Health & Social Care or willingness to work towards it

Key Responsibilities

Ensure that care worker visits are delivered in line with Hartwig Care’s commitment to continuity and flexibility.
Ensure all client visits take place on time and are allocated to the most appropriate care workers based on individual needs.
Provide care workers with all necessary and up‑to‑date information regarding client visits, service delivery expectations, and company policies.
Receive and record information and reports from care workers—both verbally and in writing—within agreed timelines.
Coordinate and schedule care worker review meetings and team meetings, ensuring they take place as planned.
Cross‑check actual visits against planned bookings to ensure accuracy and consistency in the rota.
Support ongoing improvements in service monitoring, quality assurance, and client safety.
Assist Care Managers in communicating daily updates, visit changes, and key information to care workers.
Manage incoming phone calls and text messages, taking messages and redirecting communications as appropriate through the telephone system.
