Full job description

About the Role

Good care coordination is about making sure the right support reaches the right person at the right time. Visits need to be organised, changing needs must be noticed quickly, care records have to remain accurate, and families and care teams need clear communication when circumstances change.

As a Care Coordinator, you will organise day-to-day care services for individuals receiving support at home or within community settings. You will manage care schedules, communicate with care workers and service users, monitor changes in support requirements, and help keep care delivery consistent and well documented.

The role combines scheduling, communication, and practical problem solving. One day may involve reorganising visits because of staff availability, while another could focus on updating a care plan, speaking with a family member, or coordinating additional support following a change in a service user’s needs.

Key Responsibilities

Coordinate daily and weekly care schedules for assigned service users.
Match care workers to visits according to availability, location, and support requirements.
Monitor scheduled visits and respond to cancellations or unexpected staffing changes.
Maintain accurate service-user and care-worker information.
Update care schedules when support requirements change.
Communicate regularly with service users, families, carers, and care staff.
Respond to routine enquiries regarding visits, schedules, and care arrangements.
Support the preparation and review of individual care plans.
Record significant changes in service-user circumstances and escalate concerns appropriately.
Coordinate new care packages and initial service arrangements.
Support onboarding and scheduling of newly recruited care workers.
Maintain accurate electronic care records and scheduling-system information.
Monitor missed, late, or incomplete visits and arrange corrective action.
Assist with organising additional cover during holidays and periods of staff absence.
Support communication between care teams, healthcare professionals, and community services.
Maintain confidentiality and appropriate handling of personal information.
Assist with quality checks and service reviews.
Prepare routine reports covering service delivery and staffing activity.
Identify recurring scheduling problems and recommend practical improvements.
Support safeguarding and escalation procedures where concerns are identified.

What We Are Looking For

Previous experience in care coordination, domiciliary care, healthcare administration, social care, support services, or a similar environment.
Strong organisational skills and ability to manage frequently changing schedules.
Good communication skills when working with service users, families, and care staff.
Experience using electronic care-planning or workforce-scheduling systems would be advantageous.
Ability to maintain accurate and confidential records.
Good understanding of person-centred care principles.
Experience supporting vulnerable adults or individuals with additional care needs would be beneficial.
Confidence resolving staffing and scheduling problems quickly.
Good working knowledge of Microsoft Outlook, Word, and Excel.
Understanding of safeguarding responsibilities.
Relevant health and social care qualification would be advantageous.
Ability to remain organised while handling multiple urgent requests.
