Full job description

Job Summary
We are seeking a dedicated and compassionate Patient Care Navigator to join our team. The successful candidate will play a vital role in guiding patients through their healthcare journey, ensuring they receive appropriate support and information. This position offers an opportunity to make a meaningful difference in patients' lives by providing personalised assistance within a professional and supportive environment.

Duties

Act as the primary point of contact for patients, offering guidance and support throughout their healthcare experience
Assist patients in scheduling appointments, follow-ups, and coordinating care plans
Provide information about treatment options, hospital procedures, and available resources
Liaise with medical staff to ensure seamless communication between patients and healthcare providers
Maintain accurate and confidential patient records using office software systems
Follow up with patients post-appointment to address concerns or questions and ensure ongoing support
Organise and manage administrative tasks related to patient care, including documentation and data entry

Skills

Previous office experience is highly desirable, demonstrating organisational proficiency and familiarity with administrative procedures
Excellent communication skills, with the ability to empathise and connect effectively with diverse patient populations
Strong organisational skills with attention to detail to manage multiple tasks efficiently
Ability to work collaboratively within a multidisciplinary team
Compassionate attitude with a commitment to delivering exceptional patient-centred care
Proficiency in office software such as Microsoft Office or equivalent programmes
