Job: Support Navigator (Female Only)
Hours: Starting hours 37.5 hours, with the option to work a sleep-in shift following your induction. At this point your hours will increase to 47.5. Shift patterns are either Tuesday – Saturday or Sunday – Thursday.
Salary: up to a maximum of £31,554.08 (inclusive of sleep-in) (Salary review pending for June)

As we continue to make a difference to our communities, we are looking for a Care Administrator to join the team at The Maples. As a Support Worker (or as we call them, Support Navigator), you will make a difference by providing support to individuals with Mental ill health an/ or learning disability with the objective of maximising their independence.

What will you be doing?
We are looking for a Support Navigator for our Specialised Trauma Services, which includes both regulated and non-regulated services. You will be dealing with clients who are suffering from various and complex mental health needs such as personality disorder, trauma, PTSD, self harm, bipolar disorder, eating disorder, and schizophrenia. Due to the nature of the clients you will be working with, we would require a female Support Navigator.

You will be promoting independence, providing general support, helping with the upkeep of the properties, working towards personal goals, and providing emotional support, and support with medication management.
We ensure clients can live safely, considerately and with dignity, whilst also adhering to the terms and conditions of their licence.
This is an extremely challenging role and requires individuals who have a breadth of experience of working with people who have complex mental health needs.
- Oversee the case load of service users you are responsible for to ensure all aspects of support are delivered, risks assessed and reviewed and outcomes met.
- Complete and review support plans with clear outcomes and actions that enable our service users to move towards independence and belonging.
- Promote independence and provide general support to our service users
- Complete and review clear task plans, ensuring they are followed, actioned and recorded by yourself and other members of the team.
- Complete, action, and review risk management plans whilst delivering creative, flexible and, outcomes driven support.
- Support the implementation of service users' personal goals and ensure monthly KPI targets are met and the cases you support are compliant.
- Support with crisis mitigation and resolution.
- Report on client non engagement and create strategies to improve engagement.
- Actively communicate with external healthcare professionals involved in clients support and understanding their roles and keeping them informed.
- Build strong relationships with family members and be the point of contact whilst positively representing the organisation.
- Enable service users to access and participate in their local community, including access to education and employment opportunities.
- Define & oversee individual and communal budgets and finances.
- Raise safeguarding referrals when required with relevant local authorities.
- Provide help with the upkeep of a client’s property
What will you bring to the role:
Skills and attributes required from you:
- NVQ in health and social care preferred or equivalent in experience
- Effective interpersonal skills, with the ability and confidence to generate and build effective relationships with service users
- Experience of working at Keyworker level
- Strong IT Skills
- Previous experience of health and wellbeing preferred.
- Previous experience of working within mental health
- Previous experience of working with people with trauma
- Develop and maintain an understanding of the mental health act
- Develop and maintain knowledge of medication regimes
Other Requirements:
- Enhanced DBS clearance
- A full drivers licence is required and access to your own vehicle is preferable
- Right to Work Status - there is no sponsorship available
