Keem Healthcare takes pride in providing exceptional care and support services to our valued service users, and as a Care Coordinator, you will play a pivotal role in ensuring that each individual receives personalized care tailored to their unique needs and goals are set with outcomes followed for each individual.

Job Highlights:

Conduct comprehensive assessments of clients' physical, emotional, and social needs to develop personalized support plans.

Demonstrate your expertise in content and report writing to maintain accurate and detailed documentation.

Monitor the quality of delivered care, ensure health and safety compliance, and prepare service reports.

Maintain accurate, up-to-date patient records and handle administrative paperwork.

Manage appointments, and coordinate staff/caregiver shifts and managing staff rotas.

Utilize your previous care assessment experience to identify areas for improvement and optimize support plans.

Collaborate with other healthcare providers to ensure quality care for individuals

Maintain accurate and up-to-date records of interactions with service users.

Requirements:

Proven experience as a Care Coordinator, showcasing your ability to provide exceptional care assessments.

Excellent content and report writing skills to maintain detailed and accurate documentation.

Good communicator.

Valid driving license and access to a vehicle to enable efficient travel to service users' locations.

Within commutable distance and good geographical knowledge of Coventry and Warwickshire.

Working Hours:

Flexible to meet the needs of the service.

Qualifications:

-Minimum 6 months experience as Care Coordinator
- Strong communication skills, both verbal and written
- Excellent interpersonal skills and ability to build rapport with individuals
- Knowledge of healthcare systems, policies, and regulations
- Ability to explain complex information in a clear and understandable manner
- Strong problem-solving skills and ability to think critically
- Detail-oriented with strong organizational skills
- Ability to work independently and as part of a team
