Overview
The Care Coordinator plays a vital role in ensuring that patients receive comprehensive and effective care. This position involves coordinating various aspects of patient care, facilitating communication between healthcare providers, patients, and their families. The ideal candidate will possess strong organisational skills and a commitment to providing exceptional support to both patients and healthcare teams.

Duties

Coordinate patient care by scheduling appointments, follow-ups, and necessary tests.
Serve as the primary point of contact for patients, addressing inquiries and concerns promptly.
Maintain accurate and up-to-date patient records in compliance with confidentiality regulations.
Collaborate with healthcare professionals to develop and implement care plans tailored to individual patient needs.
Assist in the management of patient referrals to specialists or additional services as required.
Monitor patient progress and ensure continuity of care through effective communication with all stakeholders.
Provide administrative support, including managing office supplies and maintaining a tidy workspace.

Skills

Previous office experience is essential for managing administrative tasks efficiently.
Strong communication skills, both verbal and written, to liaise effectively with patients and healthcare professionals.
Excellent organisational abilities to handle multiple tasks while prioritising patient needs.
Proficient in using computer systems for record keeping and scheduling.
Empathetic approach towards patients, demonstrating patience and understanding in all interactions.
Ability to work collaboratively within a team environment while also being self-motivated. This role is ideal for individuals who are passionate about making a difference in the lives of others through coordinated care efforts.

Please note that due to being a city centre Practice, staff parking is not available. This role is to be based at our main site in Great Pulteney Street, with the occasional need to work at our Bathampton site if required.

Hours required are Monday - Friday 3.00pm - 6.30pm.

Job Type: Part-time

Pay: £13.00 per hour
