Fixed Term/ Secondment opportunity for 12 months to improve service delivery.

Secondment Opportunity Existing permanent employees must first discuss this opportunity with their substantive line manager and complete the secondment request form prior to interview. We are seeking an enthusiastic, well-motivated team member to join our Discharge and Patient Flow team. As a Patient Flow Support Worker, you will play a crucial role in facilitating safe and effective patient discharges, gathering and inputting data, and ensuring smooth flow within the hospital.

Patient Flow:

Facilitate safe and efficient bed movements and discharges
Support daily bed meetings, gather and input data
Assist with system access, knowledge of local IT systems
Contribute to maintaining accurate bed states across the organisation

Discharge:

Work alongside Integrated discharge service/Ward teams to ensure safe discharges are followed by the ward.
Coordinate incoming/outgoing telephone calls and email communications between wards/departments.
Record data accurately and liaise with external partners

Requirements:

Excellent communication skills
Willingness to learn new skills
Some understanding of patient flow and safe discharge
Ability to follow instructions and work independently when required

The duties of this post require the successful candidate to be a member of the Protecting Vulnerable Groups (PVG) Scheme. More information on this scheme can be found at www.disclosurescotland.co.uk

As from 1/4/26, the Agenda for Change full-time working week will be reducing from 37 to 36 hours per week. Part time hours will be reduced pro-rata. However, there will be a corresponding increase in the hourly rate so that pay will not be affected.
