Full job description

The successful candidate will play a crucial role in supporting cancer services by managing the administrative and coordination aspects of the cancer MDT meetings. This involves ensuring accurate patient data, tracking cancer pathways to meet national targets, and facilitating communication between various stakeholders. They will join a small but friendly team based at the Hillingdon Hospital site.

Proactive and accurate tracking of patients on a cancer pathway via the cancer system, Somerset to ensure patients are seen in a timely and efficient manner.

Work closely with the multidisciplinary team to ensure patients have their 1st appointment, investigations and treatment scheduled in a timely manner.

Facilitation of multi-disciplinary team (MDT) meetings, ensuring meetings run effectively and information is collated and communicated to members.

The Hillingdon Hospitals NHS Foundation Trust is the only acute Hospital in the London Borough of Hillingdon and offers a wide range of services including accident and emergency, inpatient care, day surgery, outpatient clinics and maternity services. The Trust’s services at Mount Vernon Hospital include routine day surgery, delivered at a modern treatment centre, a minor injuries unit and outpatient clinics.

We are making progress and going forward by working in partnership with local GPs, charities, community services, academic partners, our local authority, neighbouring hospitals and the wider North West London Integrated care system, and ensuring that we listen and work in partnership with our local population. We are absolutely focused on ensuring that our hospitals provide high quality, safe and compassionate care, while drive forward the building of the new Hillingdon Hospital.

We have over 3,500 members of staff that are proud to care for nearly half a million people, with a vision to be an outstanding provider of healthcare through leading health and academic partnerships, transforming services, to provide the best care where needed.

Develop and maintain an understanding of the Cancer Waiting Times guidance to ensure that two week wait, 28 day, 31 day and 62 day targets are met

To monitor and accurately track patient on a suspected cancer pathways via the Somerset system. Ensure actions are chased and reviewed prior to the weekly cancer PTL meeting

To monitor and track ‘upgraded’ patients via Somerset

To be responsible for checking the outcomes of patients attending clinic on a suspected cancer pathway and monitor bookings of required tests within deemed timescales

To work with the team to resolve delays in the pathway at the earliest opportunity and where this is not possible escalating issues in accordance with Trust and network procedures

To assist in meeting timescales of the submission of data for national and local requirements of the Cancer Network. This includes, but is not limited to, data relating to CWT, COSD and the national cancer audits.

To assist in the validation of all mandatory CWT fields for treatment records for submission to NHS Digital within the given reporting month/quarter.

Attend the weekly PTL review meeting with the Service Manager for Cancer against the PTL SOP requirements. Escalation of issues to enable relevant actions to be taken to overcome problems with meeting targets

To assist with composing draft breach reports for all CWT treatment breaches. These should include: root cause analysis; chronology of events; recommended actions; evidence of implementation of recommendations

To assist in the running of weekly MDT follow up meetings with the MDT Lead Clinician and maintaining attendance records

To assist in the accurate input of the outcomes of the MDT meetings into the Somerset system

Assist in the preparation and circulation of lists of patients for discussion at each
multi-disciplinary team meeting (MDT) using information provided by relevant parties.

To assist with the preparation and collection of all documentation, information, medical records, imaging, and pathology required to support each meeting, thus enabling the smooth running of the MDT meeting

Where necessary to liaise with other Clinicians and MDT Coordinators/administrators outside of the Trust in relation to Tumour sites where the Trust is a Tertiary Centre

To ensure the timely and efficient transfer of patients and information between trusts involved in the patient pathway, adhering to agreed communication pathways and protocols

To assist in ensuring that all operational changes affecting the MDT are communicated effectively with the whole MDT and other interested parties

To assist in checking with the MDT Lead Clinician that all the information is correct and complete. To acquire Lead Clinician sign-off of all MDT outcomes

Assist in setting up tumour specific cancer service meetings

Use Patient Administration System to book patient outpatient appointments

Cross cover colleagues within team to ensure tracking, MDT meetings are covered

Work closely with the Team, Tumour Cancer Lead, Consultants & CNS’s. Internal teams from other departments such as Admissions, Clinical records.

Liaise with patients and carers

Establish good communication channels with external teams such as GP Surgeries/Other Trusts.
