Salary: £26,000 – £28,000 per annum (Dependent on Experience)
Hours: Full Time – 37.5 Hours Per Week
Reports To: Registered Manager
Driving Licence: Required
JOB PURPOSE
South Manchester Care Ltd is seeking an organised, proactive and professional Care Coordinator to support the day-to-day delivery of care services across Greater Manchester, Cheshire and Merseyside.
The Care Coordinator will play a vital role in ensuring care packages are staffed effectively, service users receive consistent support and care staff are deployed efficiently. The successful candidate will work closely with the Registered Manager, Field Care Supervisor and Recruitment Team to ensure the smooth operation of all services.
The role requires excellent communication, problem-solving and organisational skills, with the ability to manage multiple priorities in a fast-paced environment.
MAIN DUTIES & RESPONSIBILITIES
- Care Coordination & Scheduling
- Create and manage staff rotas using the company's care management system.
- Allocate staff to care packages based on skills, experience and service user needs.
- Ensure all shifts are covered effectively.
- Manage sickness, annual leave and emergency cover arrangements.
- Monitor continuity of care and minimise changes to staffing wherever possible.
- Ensure care visits are scheduled efficiently and in line with contractual requirements.
Service User Support
- Act as a point of contact for service users and their families.
- Respond to enquiries and concerns in a professional manner.
- Support the mobilisation of new care packages.
- Ensure service user information is accurate and up to date.
- Liaise with care staff regarding changes to care requirements.
- Escalate concerns to management where appropriate.
Staff Support
- Communicate rota information to staff.
- Monitor staff availability and working hours.
- Ensure staff are allocated appropriately according to training and competency.
- Support staff with day-to-day operational issues.
- Escalate performance concerns to management.
- Promote positive staff engagement and communication.
Compliance Responsibilities
- Monitor training expiry dates.
- Ensure staff compliance records remain up to date.
- Support DBS monitoring processes.
- Ensure mandatory documentation is completed and filed correctly.
- Monitor care note completion and visit compliance.
- Report any compliance concerns to management.
Recruitment & Onboarding Support
- Support the Recruitment Manager with onboarding processes.
- Assist with interview scheduling.
- Support induction arrangements for new staff.
- Ensure recruitment documentation is completed and stored appropriately.
- Assist with maintaining staff records.
Communication & Relationship Management
- Liaise professionally with Local Authorities, Social Workers, Healthcare Professionals and Families.
- Maintain effective communication between office staff and field-based staff.
- Support management with package reviews and service developments.
- Attend meetings as required.
Emergency & On-Call Support
- Assist with emergency staffing arrangements.
- Support out-of-hours coordination when required.
- Escalate safeguarding concerns immediately.
- Support business continuity planning during emergencies.
ESSENTIAL REQUIREMENTS
- Previous experience within the Health & Social Care sector.
- Excellent organisational and communication skills.
- Ability to manage multiple priorities and deadlines.
- Good IT skills, including Microsoft Office.
- Ability to work independently and as part of a team.
- Professional telephone manner.
- Understanding of confidentiality and data protection.
DESIRABLE REQUIREMENTS
- Previous experience as a Care Coordinator.
- Experience within domiciliary care, supported living or children's services.
- Knowledge of CQC regulations and standards.
- Experience using CareLineLive, Roundsys or similar care management systems.
- NVQ Level 2 or Level 3 Health & Social Care.
- Understanding of safeguarding procedures.
